The Hemostatic Impact Of Uterine Artery Ligation In The Cesarean Section Surgery

Objective:Explore the prevention and treatment effect of uterine artery ligation in postpartum hemorrhage.Data of clinical:Discuss42puerpera who give birth to a child by operation which have postpartum hemorrhage in YanBian University Hospital from Jan.2009--Dec.2011, all ligate the uterine artery, in the25--40, at38--41week’gestation, thereinto33samples were the first pregnant,9samples were the multipara. The other one is in the same time there are42samples who had the cesarean section but didn’t ligate the uterine artery for the matched group. In24--33, at37--42weeks’ gestation, thereinto33samples were the first pregnant,9samples were the multipara. It have no statistical significance between the age, the gestational age, the pregnant time (p>0.05). The blood coagulation time detection were all normal.Method:Compared with the total bleeding amount, the hemoglobin Before thesurgery and after the operation, the number of the postpartum hemorrhage, the lastof lochia rubra and thes situati on of menstrual after tide.Results:The amount of bleeding in the group of ligation is (225.28±28.30)ml, which is less than the matched group (316.81±87.48)ml, it has statistical significance (P<0.01). The hemoglobin aren’t different between in the two group. But the hemoglobin of ligation at before and after is5.12g/L,it is 16.21g/L in the match group(p<0.01).there are2samples have postpartum hemorrhage in ligate group compare with8samples in the match group (p<0.05). In first group the loss of blood at pregnant women who have uterine fibroids occur postpartum hemorrhage are365.00±44.91ml, the other are481.43±77.77ml7.77ml, the result have statistical significance(p<0.01). There is a woman have implanted placenta, her happened massive hemorrhage,so have the subtotal hysterectomy. All the first group are not hysterectomy. Then there are two women pregnant again in the group of artery ligation. The lochia at the first group lost5.1±2.4d, compare to8.3±2.3d, less obviously (p<0.05), it has statistical significance.The two group women have menstruation after2--9months, there no differences. Ligation of follow-up groups in84cases of patients in the two planned pregnancy and labour again, antenatal Uterine artery and umbilical blood flow resistance in the signal are normal, proved the ligation of Uterine artery recanalization after good, does not affect fetal development, does not affect fertility in the future. Ligation Group No1of84cases of uterine necrosis.Conclusion:1. postpartum hemorrhage in Cesarean pregnant women at high risk of Uterine artery ligation is effective measures for prevention and treatment of postpartum hemorrhage, effective prevention of hemorrhagic shock and keep the uterus is of great significance. 2. does not affect the ovarian function after Uterine artery ligation and fertility.3. have good collateral circulation of the uterus, Uterine artery ligation and no uterine necrosis.
