Research Of The Construction Of Evaluating Index System On The Competence Of Urban Community General Practitioner In Guangxi

Objective: Guangxi general medical career starting late,base shallow,personnel quality is not neat. Really need a set of scientific and objectiveevaluation index. But at present in guangxi province，still lack of scientific andunified GP competence evaluation index system. This study based on analytichierarchy process to explore to build evaluating index system on thecompetence of urban community GP for the fact of the Guangxi. And carrieson the empirical research.To provide a theoretical reference for governments atall levels to formulate GP related policy. And to provide reference for medicalschools in Guangxi province to develop GP student teaching programme. Theindex system can also provide scientific guidance for urban community healthinstitutions to test the performance of GP.Methods:（1）Focus group discussion: Through the focus group discussionto definition of the concept of general practitioner,research filed andparticipating experts selection criteria.(2)Literature research method: Retrieve the government department of health related documents, and literature about theGP competence evaluation.(3) Delphi expert consultation method: Based ontwo rounds of enquiry of experts who participate in the survey. This studysufficient solicit their opinions and suggestions on the construction of evaluatingindex system on the competence of urban community GP in Guangxi.(4)Analytic hierarchy process (AHP): According to the index about the GPcompetence evaluation and the logical relationship between each other, using theyaahp software to construct structured hierarchy model. Based on the modelconstruct "two comparison matrix" about internal indicators at all levels. Sendthe AHP questionnaire to expert, compare the relative importance betweeninternal indicators at all levels.(5)Questionnaire survey: According to the studybuilded the construction of evaluating index system on the competence of urbancommunity GP in Guangxi. We developed GP self-reported questionnaire.Questionnaire about part of GP on the urban community in Guangxi province.Result:(1)Through the focus group discussion to determine the object ofstudy is in the urban community work more than three years, acquire thecertificate of medical practitioners and by gp job-transfer trainingeligible.Research field is urban community health institutions in Guangxiprovince. At the same time, the selection of experts conditions were determined.(2) A provincial level for evaluating index system was established including5first–level indicators and31second-level indicators and each index weight wasalso determined. The5first-level indicators were "Basic medical service ability","Public health service ability","Humanistic practice ability","Occupationaccomplishment"," Studying and learning ability" and the indicators weightrespectively were0.2527,0.2527,0.1937,0.1818,0.2077,0.1641. Secondaryindicators "4.1abide by professional ethics","4.3improve patient satisfaction", "4.2medical psychological ability to adapt work" the top three.(3) According tothe index system to develop quantitative ability of gp self-assessment tables.Through the questionnaire survey to calculate the total Cronbach alphacoefficient of0.965. Five established hierarchy Cronbach alpha coefficient were0.808,0.871,0.882,0.832,0.839explain reliability is good. Questionnaire surfacehas good validity and content validity, structure validity is poorer.Conclusions: This study explores to build a set of scientific, suitable andconvenient operation of the construction of evaluating index system on thecompetence of urban community GP in Guangxi. The first–level indicators"Basic medical service ability" weight value first reflect the medical service isthe core ability of GP. The second-level indicators "4.1abide by professionalethics","4.3improve patient satisfaction","4.2medical psychological ability toadapt work" belong to "Occupation accomplishment" category reflect thegeneral practitioner in daily work should make great efforts to improve theprofessional quality, shape good professional image. And "education learningability" scored lowest reflect the index is not the focus of the GP competence.Based on the resluts of GP self-reported questionnaire, the questionnairereliability and validity of basic meet the requirements, suggests constructing theindex system in Guangxi province has certain reference significance.
