| Objectire:Assessment of HER-2protein/gene status and chromosome17ploidy in gastric cancer and their clinical pathological characteristics, to provide theoretical basis for the disease mechanism and molecular targeted therapy on gastric cancer.Methods:The expression of HER-2protein in115cases of gastric biopsy specimens was detected by immunohistochemical method(IHC),and the samples were further by using fluorescence in situ hybridization(FISH)to analysis the HER-2gene amplification ststus and the relevant chromosome17aneuploidy.Results:(1)in115cases of gastric carcinoma,the expression of HER-2protein (IHC score-,+,++,+++)were34,35,25and21,the positive expression rate of HER-2protein(IHC score of++and+++)was40%(46/115).(2)the positive rate of HER2protein expression is related to Lauren’s type, degree of differentiation, invasion depth,1ymph node metastasis and clinical staging, metastasis distant degree(p<0.05),no significant correlation with the factors of patients’ age and sex,tumor size and tumor location(p>0.05).(3)HER-2gene amplification in the IHC score(-/+),(++),(+++)in three groups were1.4%(1/69),28%(7/25)and90.5%(19/21),the general amplification rate is23.5%(27/115).HER-2gene amplification in IHC(+++)group was higher than that of IHC(++)and IHC(-/+)group,HER-2gene amplification rate between the three groups had significant difference(p<0.05).In the IHC(-/+), IHC(++)and IHC (+++)three groups,The coincidence rate of IHC and FISH were98.6%(68/69),28%(7/25)and90.5%(19/21),The coincidence in IHC(++)group is low,in (-/+)and(+++)in line with the higher rate.(4)chromosome17polysor HER-2protein expression group(++/+++)and low expression group(-/+)19cases and5cases,the incidence of chromosome17polysomy expression group was higher than that of low expression group (41.3%,19/467.2%,5/69), the difference was statistically significant (p<0.05). In Group FISH (+),The HER-2gene amplification is13cases(48.1%,13/27); In Group FISH (-) is11cases(12.5%,11/88). The rate in FISH (+) group was higher than that of FISH (-), the difference was statistically significant (p<0.05).Conclusion:1. In115patients,the cases of HER-2overexpression is46, the overexpression rate was40%.2.The HER-2protein overexpression have correlation with the pathological differentiation, Lauren’s type, degree of differentiation, invasion depth, lymph node metastasis, distant metastasis and clinical stage and other factors, was not associated with the patient’s age, gender factors, tumor size, tumor location etc..3.In115cases of gastric carcinoma,the HER-2gene amplification is27, the overall amplification rate is23.5%(27/115).The HER-2gene amplification have correlation with the HER-2protein expression(p<0.05). On IHC (++) group, IHC and FISH by coincidence rate is low, need to do FISH testing of HER-2gene amplification4. Chromosome17polysomy rate in HER-2protein overexpression group was higher than that of low expression group, the rate of HER-2gene amplification group was higher than that of none amplification group, suggesting that chromosome17polysomy possibly associated with overexpression of HER-2protein and HER-2gene amplification, chromosome17polysomy may serve as an indicator of the development and prediction the prognosis of gastric cancer. |
