Objective:To explore the impact factors of esophageal precancerous lesions in Huaian District, Huaian City, Jiangsu Province, where is a high-incidence area of esophageal cancer, and to understand the relationships between serum folic acid、VB2、VB6、VB12levels and esophageal precancerous lesions, then further to know the relationship between MTR A2756G polymorphism and esophageal precancerous lesions risk. These studies provided the scientific basis for prevention and treatment of esophageal cancer with folic acid、VB2、VB6、B12and carrying out prevention and treatment of esophageal cancer in Huaian District, and in turn to reduce incidence rate and mortality of esophageal cancer.Methods:(1) Our study selected the esophageal cancer high-risk area in Huaian District. Relying on the ministry of health’s project of the early detection and early treatment, using random cluster sampling method select study. objects for epidemiological survey and gastric endoscopic screening, the study objects from two local villages and the age from40to69.Our study compared and analyzed the esophageal precancerous lesions cases and part of normal groups’the questionnaire dates. The two groups from the whole group, through the endoscopic screening in2011.The questionnaire include:the basic situation, disease history, diet history, family history of cancer, smoking history, drinking history of research subjects. Based on the survey, logistic regression analysis about impact factors of esophageal precancerous lesions was conducted.(2) The enzyme-linked immunosorbent assay was used to observe the serum folic acid、VB2、VB6、VB12levels and comparing the difference between esophageal precancerous lesions cases and control group. Then the serum vitamin which was significantly different between two groups was divided into four levels according to control group’s vitamin level as well as the OR value of esophageal precancerous lesions among different levels were estimated.(3) MTR genotypes at the A2756G site were analyzed by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) methods. Chi-square test was used for estimating MTR genotype frequency distribution in esophageal precancerous lesions cases and control group.Results:(1) Unconditioned Logistic regression analysis showed that the, eating fast, eating hot food, low average annual income, history of digestive system diseases were the possible risk factors of esophageal precancerous lesions in Huaian District.(2) The level of folic acid in serum of esophageal precancerous lesions group were significantly lower than normal control group (P<0.05), and the OR values of the highest quarter serum folic acid compared with the lowest quarter for esophageal precancerous lesions occurrence were0.348(95%CI:0.174-0.693), but the levels of VB2、VB6、VB12in serum of esophageal precancerous lesions group had no significantly different with normal control group.(3) MTR A2756G genotype and allele distributions in normal control group and esophageal precancerous lesions group had no significant differences (P>0.05), and MTR gene polymorphism has no statistical correlation with the disease susceptibility of esophageal precancerous lesions.Conclusion:(1) Eating fast, eating hot food, low average annual income, history of digestive system diseases were the possible risk factors of esophageal precancerous lesions in Huaian District. Some strategies such as changing bad eating habits, dietary structure and lifestyle can be used to prevent the occurrence of esophageal precancerous lesions. At the same time the high-risk residents who had history of digestive system diseases (including family history of cancer) must be monitored in order to find patients of esophageal cancer as early as possible and reduce the incidence esophageal cancer.(2) The serum folic acid deficiency was the possible risk factor of esophageal precancerous lesions. It may be can through taking food that contain rich folic acid from abundant food or vitamin supplements to prevent esophageal precancerous lesions,MTR A2756G polymorphism may have no correlation between the risk of esophageal cancer.
