Objective:To evaluate the efficacy and safety of Jingxin Zhidong Fang in treating moderate and severe tic disorders; To explore the accommodation of serum levels of IgGl, IgG2and IgG3by Jingxin Zhidong Fang; To observe the deficiency of IgG subclass in moderate and severe tic disorders; To investigate correlations between IgG subclass concentrations and symptom severity.Methods:A prospective study was conducted.29patients with moderate and severe TD were recruited at the Pediatric Clinic of Guang’anmen Hospital, China Academy of Chinese Medical Science. Patients received no Hormones, immunotherapy and TD control drugs in six months before herbal treatment. All patients received herbal treatment of Jingxin Zhidong Fang continuously for three months. We compared the results of The Yale Global Tic Severity Scale (YGTSS) before herbal treatment versus three months after treatment. Their liver-kidney functions was monitored before and after treatment.The serum of all patients was collected at the first visit and the endpoint visit of the research. All serum samples were stored in a-80℃freezer until assay. The assay results were compared with13patients without TD. Serum levels of IgGl, IgG2and IgG3were tested by enzyme-linked immunosorbent (ELISA). Experimental data were statistically analyzed.Results:1. After3months treatment by Jingxin Zhidong Fang, the total effective rate was76%. A total of29patients with moderate and severe TD, significantly improved in13cases, improved in9cases and remained in7cases. The total effective rate was92%in severe TD, which was higher than moderate TD. Liver-kidney functions was all normal in29patients before and after treatment.2. After3months treatment by Jingxin Zhidong Fang, serum levels of IgGl were significantly higher than before the treatment (P=0.0030.05); The rate of deficiency of IgGl was44.8%down to20.7%after3months treatment (P=0.0460.05).3. The rate of deficiency of IgGl was significantly higher in patients with moderate and severe TD than in patients without TD (P=0.045<0.05); The rate of deficiency of IgG2was significantly lower in moderate and severe TD than in patients without TD (P=0.0270.05); No differences in the rate of deficiency of IgG3were identified between two groups (P=0.163>0.05).4. We compared the levels of IgG subclass in patients with moderate TD and severe TD, no differences were present; There were no statistically significant linear correlation between levels of IgG subclass and symptom severity.Conclusion:The clinical efficacy and safety of Jingxin Zhidong Fang in treating moderate and severe tic disorders was reliable under the condition of this study. The clinical efficacy and improvement in patients with severe TD was better than patients with moderate TD; Jingxin Zhidong Fang could improve the serum level of IgG1; Significant differences were found in the rate of deficiency of IgG1and IgG2between the patients with TD and patients without TD; There were no correlations between IgG subclass concentrations and symptom severity.
