ObjectiveThe etiology and pathogenesis of Non-alcoholic Fatty Liver disease (NAFLD) is complicated,and chinese physicians to the disease pathogenesis understanding is different,lacking of drugs and theoretical innovation, currently,modern medicine is still a lack of effects of drug treatment of the disease, therefore, according to the clinical experience of Professor Fu Si treatment of Non-alcoholic Fatty Liver disease,with the principle nourishing liver and spleen,dispersing plegmn and removing blood stasis method,the Decotion of Jingtiantiaogan by this method was established to treat NAFLD.The clinical research part based on the observation of clinical effect on treatment of Decoction of Jingtiantiaogan for the liver depression and spleen deficiency syndrome type of NAFLD,in order to investigate the traditional Chinese medicine theory in the treatment of NAFLD,and to valuate objectively its effectiveness and safety of the treatment of NAFLD,to provide an objective basis for clinical application,has certain theoretical significance and clinical value.MethodReferring to the Institute of Chinese Medical Association liver disease fatty liver and alcoholic liver disease study group "of non-alcoholic fatty liver disease diagnosis and treatment guidelines" in the non-alcoholic fatty liver disease diagnostic criteria and traditional Chinese medicine(TCM) diagnosis the liver depression and spleen deficiency syndrome type,combining with the tutor’s clinical experience for clinical investigation form,102cases of patients who met the inclusion criteria,undergone health educationrpaid attention to diet,had the appropriate physical exercise,regulated emotions etc.The patients were randomly divided into TCM group and the control group51patients,fall off in5cases of each group,completed in92cases with46cases in each,in the therapy of10weeks,patients in the two group by gender,age,body weight,BMI,severity of patient’s condition,the symptom integral etc,there was no significant differene (P>0.05),and comparability.The control group adoption VitE soft capsule,TCM group take VitE soft capsule and the Decoction of Jingtiantiaogan.At the end of treatment,observing the clinical total effective rate,clinical symptoms and signs,liver function(AST,ALT,ect),lipid(TC,TG,ect) and Liver B-imaging, accordi-ng to the related standard,make diagnos is effect assessment.Results 1.On the overall efficacy,clinical total effective rate of TCM group was84.8%,better than the control group for65.2%(P<0.05) suggesting that Decoction of Jingtiantiaogan for the liver depression and spleen deficiency syndrome type of NAFLD has better effect in the treatment.2.After treatment,reducing body weight and body mass index of the two groups,in the treatment group compared with control group,significant differences (P<0.01)3.In the TCM symptoms,Hypochondrium pain,abdominal distension,fatigue,fi-dgety and irascibility,sigh,loose stools,abdominal pain for diarrhea of two group compared with before treatment were significantly improved (P<0.01). Hypochon-drium pain of TCM group,better than the control group (P<0.01). Fidgety and i-rascibility,loose stools,abdominal pain for diarrhea of TCM group,better than the control group in some certain (P<0.05).Inappetence of TCM group was significant improvement (P<0.01),better than the control group.4.In depressing ALT and ALP,TCM group was better than the control group (P<0.01),for GGT,differences was in certain (P<0.05),for AST,the difference was not obvious (P>0.05)5.In depressing TC and elevating HDL-C,TCM group was better than the control group (P<0.01),in depressing TG,LDL-C,differences was in certain (P<0.05)6. In the Liver B-imaging,recovery rate of TCM group was45.65%,the control group was21.74%,with rank sum test,TCM group was better than the control group (P<0.01)7.The majority of patients in the TCM group was smooth defecation,after consciously relaxed,stool before forming and quality viscosity decreased,not accompanied by obvious discomfort.Conclusion1.The clinical effect on treatment of Decoction of Jingtiantiaogan for the spleen-liver deficiency of NAFLD is abvious, which was established、satisfied and no adverse effect after long terms.2.With the principle nourishing liver and spleen,dispersing plegmn and removing blood stasis method,the Decotion of Jingtiantiaogan was established to treat the liver depression and spleen deficiency syndrome type of NAFLD,which is agreed with concept of TCM,we should regulatory function in accordance with the theory of interlinked communicant as well as the interior-exterior relationship between the corresponding zang and fu organs,providing a new thought and method for clinical application.
