The medical insurance system is in order to solve the problem of national prevention and treatment of disease and raise, distribution and use of medical insurance fund in accordance with the provisions of the insurance contract. Medical insurance system is a progressive social security system, which is the inevitable result of the positive development of the progress of human civilization and society. It has a irreplaceable role in maintaining social stability and ensuring the healthy and sustainable development of the social economy.At the same time, the reform of the medical insurance system has been an important part of the medical and health system reform in China. It is an indispensable part of the "three medical interaction". Its future reform and development direction in relation to the success or failure of the "new medical reform".Japan is not only the first country in Asia to implement health insurance, but also one of the countries in the world to establish a health insurance system earlier than others. In the medical insurance system of the management mode early imported from Germany, after the Second World War learned from the certain practices in the United States, and gradually developed the implementation of a series of health legislation and relevant laws and regulations, eventually formed the unique characteristics of the Japanese medical insurance system. It has played a pivotal role in maintaining the sustainable development of the Japanese economy, ensuring the stability of the society and the stability of the society.At present, China’s health care reform is into deep water, and the reform of the medical insurance system also faces many challenges. While the Japanese medical insurance system in the process of development, has encountered or is actively coping with the problems of many problems that china has a similarity, and a lot of reform measures have achieved remarkable results. At the same time, Japan and China belong to the Confucian culture circle, which is a country with a long history of agricultural society. It has similarities and similar it both in the cultural origins and economic and social evolution path. Therefore, it makes use of the related methods and measures of the reform of medical insurance system in Japan to promote the development and progress of the medical insurance system in China.By compared with historical evolution and specific projects, the focus of the study is the economic effect of the medical insurance system. Based on push-pull effect model and medical insurance system effect consumption model of medical insurance system, make empirical analysis on China and Japan economic effect. Draw the conclusion on it and try to put forward policy suggestions on the reform and improvement of China’s medical insurance system.In addition to the introduction, this paper is divided into five chapters, the specific content is as follows:The first chapter introduces the related concepts and theories. This chapter emphatically research several important concepts which need specifically defined. At the same time, public products and market failure theory, adverse selection and moral hazard and consumption theory related basic theory are discussed.In the second chapter, through the longitudinal comparies on with Japan on the development of medical insurance. This chapter in history, operation status and evolution path, compare with the Japanese medical insurance system from the vertical. Draw the outline of the overall framework their medical insurance system based on the overview of health care system development process.Finally, find advantages and disadvantages from the system evolution path between China and Japan.In the third chapter, make a horizontal comparison on the medical insurance system between China and Japan through the specific project. This chapter, for further on the current medical insurance system in China and Japan were compared and analyzed, selecting five specific projects---the insurer, the insured object, fund-raising system, payment system and balance, making comparative study through different items and perspective. Based on the similarities and differences in the same project, it is found that the characteristics and innovative points of Japan which can solve China’s problems and deficiencies.The fourth chapter is the core, which is the empirical analysis of the economic effect of the medical insurance system in China and Japan. Taking into account the author’s own practical ability, this paper will mainly focus on the macro level of research and analysis. Here, in order to facilitate the research and calculation, the article will be divided into two parts which includes the effect of economic development on the national economic level and the consumption effect of residents on the individual level. For the research and analysis of the effect of economic development, this paper will use the "push pull effect" model to calculate, and Specific indicators includes deeper capita GDP and medical insurance fund expenditures, investigating the influence of medical insurance system on the development of national economy. And for the research and analysis of the residents’ consumption effect, the paper will build the model of the medical insurance system to carry out the specific calculation, and Specific indicators includes the level of domestic consumption, domestic health insurance expenditure per capita, the per capita wealth of the country’s residents and their dependency ratio, investigating the impact of medical insurance system on the consumption level of residents. Through the open data in the relevant literature and the official website, the comparison of the economic and social effects of the Sino Japanese medical insurance system calculated, and the results of the analysis of the two countries are briefly described.The fifth chapter, puts forward the conclusion and relevant suggestions. In view of the existing problems, according to measurement data, reform of the medical insurance payment system are proposed.The aim is to promote the rational allocation of health resources, comprehensively separate the medica land health care,revitalize health insurance precipitation funds and accelerate the improvement of the related system reform and specific recommendations.
