Objective: To analyse the overall treatment effect and clinicalsignificance of the small bone window with the side of the road wascracked by microsurgery and traditional temporal craniotomy of opera-tion of the surgical hematoma two methods for treatment on hyperten-sive intracerebral hemorrhage in or nearby the basal ganglion.Methods: A total of92cases with hypertensive intracerebrelhemorrhage hospitalized in neurosurgery department of Fujian MedicalCollege attached Union Hospital and the Second Hospital of Longyanbetween September2008and September2010were subjected in this stud-y. The cases were diagnosed by CT scanning of the head and with sur-gical treatment. They were divided into two groups according to thedifferent surgical methods: Group I (n=46),receiving craniotomy withsmall bone by lateral fissure and Group II (n=46)receiving routinecraniotomy group.The differences of the mortality rate,pulmonary in-fection, the rebleeding, the short-term and long-term prognosis andthe quality of life were compared with two groups.Result:The mortality rate, rebleeding rate and the postoperativecomplication rate in group I were6.5%,0%,13%respectively. The mor-tality rate, rebleeding rate and the postoperative complication ratein group II were10.9%,2.1%,28%.There was no significant statisti-cal difference between the two groups. But their GOS fitneness andscore of activities of daily living(ADL)were investigated during the6months’close follow up, GOS and ADL in group I is much higherthan the other one. Conclusion:To hypertensive cerebral hemorrhage in basal ganglion,especially whose volume of hematoma is between40-60ml,the microsurge-ry treatment by small skull fenestral is accord with the basic requi-rements of minimally invasive neurosurgery, it clears the hematomacompletely,it can obviously improve the neurological function, theshort-term and long-term prognosis, and the life quality of thepatients.
