Objective:The high incidence of craniocerebral injury(CI)and sequela,but sofar thereis no effective treatment methods. This topic through the observation ofhuman embryonic neural stem cells(ESCs)transplantation in the treatment ofcraniocerebral injury sequelae in recent curative effect, preliminary study ofstereotactic neural stem cells transplantation in the treatment of craniocerebral injuryvalue.Methods:Choose our department from December2010to December2013,33cases of frameless stereotactic neural stem cells transplantation treating sequelae ofcraniocerebral injury patients as the research object.The strict screening for transplantoperation conditions of the patients, preoperative laboratory examination andelectrocardiogram, chest X-ray, head CT or MRI examination with2mm scans anddisc.After preoperative preparations,the patients into the operation room,under localanesthesia or general anesthesia, adoption frameless stereotactic technique,humanembryonic neural stem cells transplantation in patients with brain lesion,to observe thenumber of white blood cells before and after operation,the change of liver function、renal function、electrolyte,postoperative side effects occur.After6months for patientswith Glasgow coma scale(GCS),to evaluate the changes of muscle strength、muscletension and functional independence measure(FIM),collect statistical related data,the application of SPSS17.0statistical software package to analyze the above databefore and after the transplantation operation.Results:33cases of stereotactic neural stem cells transplantate operation success rate is100%,postoperative24h head CT scans no intracranial hemorrhage and otherabnormal signs。Preoperative Glasgow coma scale mean score is (7.23±1.11),after sixmonths of Glasgow coma scale averages is (9.34±1.32),postoperative Glasgow comascale increase significantly (P <0.05).There are18cases in23cases of male patientspostoperative symptoms eased,the remission rate is78.3%, seven of the ten femalepatients with symptoms eased,the remission rate is70.0%,less than men,but thedifference is no statistically significant (P>0.05),and3~10years old patientsrelieve rate is higher than other age groups,the difference is statistically significant (P<0.05).Patients with craniocerebral trauma sequela before surgery,6months aftersurgery, FIM score,(P <0.05).Through the analysis of these33patients before andafter transplantation of white blood cells, liver function, renal function and electrolytechanges that: the average number of white blood cells after transplantation is [(9.3+2.7)×109]/L,significantly higher than the before transplantation level [(6.6+1.1)×109]/L,the difference is statistically significant (P <0.05),and liver function, renalfunction and electrolyte has no obvious change before and after transplantation(P>0.05).6months after transplantation,29cases of patients with high muscle tension in18patients decreased muscle tension,the muscle tension is normal in3cases,part ofthe decline in15cases,muscle tension down at a rate of62.1%.Muscle strengthimprove after operation with20cases of patients,patients with preoperative musclestrength averaged (2.14±1.01),postoperative muscle strength averaged (3.53±1.12),postoperative strength score higher than preoperative,the difference is statisticallysignificant (P<0.05).Postoperatively some patients with fever, runny nose, headache,head bilges, nausea and other symptoms,but the symptoms are mild,to givesymptomatic treatment eliminated in2~5days.Conclusions:1. Neural stem cell transplantation in the treatment of craniocerebraltrauma sequela recent curative effect, safe and reliable,adverse reactions toslightly,no obvious rejection occurred.2. Neural stem cells transplantation can be a certain degree of repair,improve patients with the lack of nerve function loss,to promote the improvement of clinicalsymptoms in patients with traumatic brain injury,improve the quality of life,toencourage the patients return to society as soon as possible.3. Neural stem cells transplantation will be like endoscopic neurosurgery, microneurosurgery, radiation neurosurgery and intervene in neurosurgery,which wouldbecome the main technology and development direction of neurosurgery.4. With regard to clinical application of neural stem cells transplantation, there arestill many problems to be solved,but extremely attractive,has shown bright, attractiveprospect.5. Neural stem cells transplantation can improve the early clinical effect,but thelong-term effect still needs further observation.
