objective To study on the tracheal mucosa cilia damage,the lung tissuepathology change and the expression of IL-6protein in the lung tissue afterintratracheal instillation with levofloxacin injection,which is in orderto understand the injury of rat tracheal mucosa and lung tissue and to findthe protective effect with injection of ambroxol hydrochloride.methods Randomly divided the90clean healthy male SD rats into threegroups, normal control group(30rats),levofloxacin group(30rats)andlevofloxacin plus ambroxol hydrochloride intervention group(30rats)respectively.In the levofloxacin group and levofloxacin plus ambroxolhydrochloride intervention group,inject the levofloxacin injectiondisposable and slowly into intratracheal through thyrocricoid puncture withdose of2.8mg per100g after the rats received intraperitoneal anesthesia.Inlevofloxacin plus ambroxol hydrochloride intervention group,Inject theambroxol hydrochloride into the abdominal cavity with dose of70mg per1Kg6hours before levofloxacin hydrochloride into trachea.They were sacrificedrespectively with5rats at the time point of2hour、1day、2day、3day、5dayand7day after intratracheal instillation,then cutting the trachea frommiddle-lower section and the posterior leave of right lung.Detect the damageof the trachea cilia and lung tissue by the electron microscope and the lightmicroscope respectively.Detecte the expression of IL-6protein usingimmunohistochemical method.Results1.the injury of tracheal mucosa cilia1.1Cilia rank were in order and density,no lodging,no edema,no fallsoff and necrosis were observed under the electron microscope in the normal control group, in levofloxacin and ambroxol hydrochloride interventiongroups,the cilia at each time point in different level of lodging, edema,necrosis and falls off except the7th day.1.2Compare to the normal control group,the mucosa cilia was damagedin different degree at2th hour,1th day,2th day,3th day and5th day inlevofloxacin group,which had great difference (P<0.05).1.3In the levofloxacin group, As time progress,the damage aggravatinggradually,and the trachea damage reached the serious at the3th day,thedifferences were significant difference between the different time points(P<0.05).1.4Compare to levofloxacin injection group,the cilia damage have beenreduced at the2th hour,2th day,3th day and5th day (P<0.05)in ambroxolhydrochloride intervention group.2. The pathological change of lung tissue2.1The tracheal and bronchial epithelium cell was in order,there wasno exudation in the bronchial lumen when observed under the lightmicroscope.In levofloxacin group, bronchial epithelial cells damagedifferently,pulmonary alveolus were infiltrated by inflammatory cells,Red blood cells and inflammatory cells infiltrated into the alveolarlumen..It return to normal at the7th day.2.2Compared with the lung tissue damage degrees at each time point inlevofloxacin group,the1th day was significant serious than the2thhour(P<0.05),the2th day turn to the most serious and the7th day almostreturned to normal.2.3Compared with levofloxacin group,the damage was reduced on ambroxolhydrochloride intervention group at the2th hour,2th day,3th day and5thday(P<0.05).3.the express change of IL-6 3.1There showed the express of IL-6small amount in lung tissue mainlyat the bronchial epithelial cell cytoplasm in the the normal control groupand it stained light yellow.Compared with normal control group,thelevofloxacin group appeared significant difference (P<0.05)at each timeexcept the7th day.3.2In the levofloxacin group,most of the bronchial epithelial cellcytoplasm are colored brown at the1th day and it maintain3day.At the5thday the color become lighter and turn into light yellow at the7th days.3.3Compared with levofloxacin group,the express of IL-6significantreduced(P<0.05)in ambroxol hydrochloride intervention group except the1thday and7th day.Conclusion1.Intratracheal instillation of levofloxacin injection could cause ratof the tracheal mucosa cilia and lung tissue damage and the increase expressof IL-6protein in lung tissue,which may involved in the process of damage.2.Ambroxol hydrochloride can make some protective effects on the airwaymucosa,lung tissue and reduce the expression of IL-6protein in the lungtissue.
