| Objective:Different age groups were retrospectively analyzed，we study the etiology ofatelectasis characteristic and microscopic features, it has significant value inbronchoscopy technique of the diagnosis and treatment for atelectasis, which provideatelectasis patients with the early diagnosis and normal treatment。Methods:150cases of patients with clinical data were collected from yanan universityhospital respiratory medicine inpatient and outpatient(January2012-June2013),whichdiagnosed by chest radiographic examination。First，check the routine electrocardiogram、blood routine、five blood routine examination、Clotting series and so on,，determining thepatients without bronchoscopy contraindications, giving upper airway anesthesia supine、administering oxygen inhalation through nasal catheter、 all operate downwardcardiopulmonary monitoring。According to the microscopic lesions in different parts andmorphology, taking brush biopsy、forceps check、irrigation of one or more inspectionmethods, such as to make a diagnosis and give treatment, postoperative samplesinspection, explicit the pathological diagnosis and further treatment。Results:1.The incidence of atelectasis in different age groups as follows：young incidence(16.0%)、the middle-aged group(22.7%)、elderly group(61.3%), and that along with theage growth, increase the incidence of atelectasis。2.Different gender of the incidence in atelectasis： women (52.0%)、men (48.0%),that women the incidence of atelectasis in rising trend。3.Different parts of the incidence of atelectasis in turn as follows: the main branch(1.3%)、the upper left lobe (22.7%)、 the left lower lobe (11.3%)、the right main branch(2.0%)、upper lobe (18.7%)、middle (34.0%)、lower lobe (10.0%), that the right lung (64.7%) higer than the left lung (35.3%),of which the middle of the right lung is in thelead。4.Atelectasis pathogeny：tumor (38.0%)、inflammation (26.7%)、dust (22.0%)、tuberculosis (6.0%)、sputum bolt (4.7%)、others(2.7%), it shows tumor、inflammation、dust is the three main causes of atelectasis。5.Atelectasis pathogeny in different age groups as follows：the elderly groupatelectasis in the majority with tumor (41.3%)、dust (33.7%)，middle age group ofatelectasis in the majority with tumor (44.1%)、inflammation (35.3%)，youth in themajority with inflammation (41.7%)、tuberculosis (25.0%)，and an analysis of the elderlygroup tumor occurrence rate relatively high youth of atelectasis (40.8%vs16.7%,chi-square=5.048, p=0.025), tumor resulted in the incidence of atelectasis in theelderly group and young group the difference was statistically significant。6.Atelectasis in different parts of the common causes：tumor (59.7%) are morecommmon in double upper lobe atelectasis；Middle of atelectasis is given priority to withinflammation (49.0%) and dust (33.3%)；Lower lobe atelectasis in inflammation (21.9%)of traumatic atelectasis caused by sputum bolt sex (18.8%)；tuberculosis occurs in upperlobe and lower lobe (8.5%).7.Gender atelectasis common cause in the order: men in the majority with tumor(64.8%) and dust (19.7%), women with inflammatory(39.3%) and dust (24.1%) were themost common。Conclusion:1.The incidence of atelectasis increased gradually with the age；the incidence oftumor also gradually increased with the age, so the young and old patients withatelectasis (especially men) should be underwent bronchoscopy clear etiology diagnosisas soon as possible, the re-inspection when necessary, in order to avoid missed diagnosis。2. The right lung higer than the left lung，of which the middle of the right lung is inthe lead。the elderly group atelectasis in the majority with tumor、dust，middle age group of atelectasis in the majority with tumor、inflammation，youth in the majority withinflammation、tuberculosis，and an analysis of the elderly group tumor occurrence raterelatively high youth of atelectasis。3. Atelectasis in different parts of the common causes：tumor are more commmonin double upper lobe atelectasis； Middle of atelectasis is given priority to withinflammation and dust；Lower lobe atelectasis in inflammation of traumatic atelectasiscaused by sputum bolt sex；tuberculosis occurs in upper lobe and lower lobe。4. Women atelectasis pathogeny is given priority to with dust and inflammationand other benign lesions, male is given priority to with tumor And women the incidenceof atelectasis has a rising trend。... |
