Pancreatic cancer is a highly malignant tumor of the digestive system. The early symptoms are not obvious so that most patients have lost the opportunity of radical operation of pancreatic cancer when abdominal distension,abdominal pain and jaundice are appeared, because the local blood vessels,viscera invasion and lymph nodes had appeared, and the5years survival rate was3%-5%. In recent years, along with the development of molecular biology, improvement of diagnosis and treatment technology,breakthrough should be made, new atherapeutic target of tumor must be done to the treatment of pancreatic cancer,so that specific individualized treatment of pancreatic cancer must be realized.Fundamental reasons for occurrence and development of pancreatic cancer is that a change in gene expression when explained from molecular level, such as the activation of oncogene, inactivation of tumor suppressor genes.SATB1(special AT-rich sequence binding protein1, SATB1) is a nuclear matrix binding protein which involved in formation of chromatin high-grade structure, development of thymus cell and maturation of T cells. Han firstly reported the relationship between SATB1and tumor invasion in the’nature’in2008. Breast cells acquire invasive ability when the genes were transplanted. Multivariate analysis showed that, the expression of SATB1is an independent prognostic indicator of breast cancer from tumor staging, histological staging and lymph node metastasis.Studies have showed that SATB1also expressed in colorectal cancer, lymphoma, gastric cancer, nonsmall cell lung cancer and other malignant tumors, but it has not related reports stating the relationship between SATB1expression and pancreatic tissues.Objective:To detect the expression of SATB1in pancreaticductal adenocarcinoma (PDAC), adjacent to carcinoma tissue and normal tissue,and investigate the relationship between the expression of SATB1and the clinicopathological factors of pancreatic ductal adenocarcinoma(PDAC). The postoperative survival time was followed up, to discusses the expression level of SATB1and postoperative survival,and analysis the relationship that the expression level of SATB1with the prognosis of patients.To provide new ideas of the mechanism of the occurrence,development of clinical treatment and prognosis judgment of pancreatic cancer.Method:Collect la samples49cases the pathological diagnosis of pancreatic ductal adenocarcinoma between January2007and December2012in tianjin nankai hospital. The expression of SATB1protein in49paired paraffin embedded PDAC speciments,western blot and RT-PCR were used to examine the expression of SATB1protein and mRNA in16paired fresh PDAC speciments and adjuvant non-cancerous pancreatic tissues.And record that whether tumor metastasis and recurrence and survival by visiting the outpatient and telephone follow-up of primary tumor patients after radical operation. Analyzed the relationship between SATB1and both the clinicpathologcial factors and prognosis of the PDAC patients by using x2test and Kaplan-Meier survival analysis.Cox proportion hazard regression analysis was used to study the independent prognostic factors.Results:SATB1expression was much higher in36PDAC tissues than that in paired adjuvant non-cancerous pancreatic samples (t=2.237, p=0.022).SATB1was over expressed in13PDAC tissues,but only5adjuvant non-cancerous pancreatic tissues (26.5%vs.10.2%).The expression of SATB1related with differentiation (x2=4.131, p=0.042),lymph node metastasis(x2=5.000, p=0.025),T stage (x2=14.348, p=0.001) and TNM stage (x2=6.69,p=0.031).The median survival time was353days in post-operation patients with positive expression of SATB1, while508days in patients with negative expression.The expression of SATB1mRNA was higher in PDAC than that in non-cancerous tissues detected by RT-PCR (t=2.432, p=0.028),but no significant difference was found in protein level (t=0.324, p=0.221).The patient’s survival time was statistically correlated with the SATB1expression (p<0.001).TNM stage (RR=36.969,95%CI:5.908-270.386) and SATB1expression (RR=0.133,95%CI:0.032-0.552) were independent prognostic factors in patients with PDAC (p<0.05)Conclusions:1. Expression of SATB1in PDAC tissues was significantly higher than that in tissue adjacent to cancer, suggest that SATB1may play an important role in the occurrence and development of PDAC.2. The high expression of SATB1is associated with difference degree, lymph node metastasis, T staging and TNM staging.3. The positive expression of SATB1in PDAC patients with a median survival time (353d) are much smaller than the negative expression (508d).4.Multivariate analysis showed that TNM stage and expression of SATB1is an independent prognostic factor in patients with pancreatic cancer (p=0.006). |
