Objectives:To investigate the relationship between Proprotein convertase subtilisin/kexin type9(PCSK9) E670G polymorphism,lipid metabolism and coronary heart disease(CHD) among the Chinese Han population of the Tianjin area; Compared the therapeutic effect of torvastatin in different PCSK9E670G genotype in patients with coronary heart disease.Methods:1. Select778patients who were admitted to coronary angiography in Tianjin Chest Hospital from Sep2010to Oct2011.According to the results of coronary angiography classified into CHD group, n=484,normal control group,n=294.Application of polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) method to the total research object E670G gene loci polymorphisms were analyzed, Analysis of the E670G PCSK9polymorphism and lipid metabolism and the correlation of CHD risk.2. CHD patients admitted to hospital for atorvastatin calcium20mg, every night. After treatment for3months, observation of statins to carry the PCSK9E670G different genotypes of coronary heart disease in the blood lipid regulation.Results:l.The frequencies of A and G allele in the Tianjin Chinese-Han healthy population were90.7%and9.3%respectively, A allele was lowerer than that in other domestic and foreign populations.2. AA, AG, GG genotype between patients with TC and LDL-C,showed a trend of rising, AA genotype in comparison with AG genotype,the difference did not reach statistical significance (P>0.05),But compared with patients with AA genotype, GG genotype carriers had a tendency to a higher level of TC, LDL-C, the difference reached statistical significance (P<0.05).3. PCSK9E670G polymorphism was related to the incidence of CHD risk,may be one of the genetic risk factors among the Chinese Han population of the Tianjin area. The frequencies of A and G allele in patients with CHD group were84.0%and 16.0%respectively, G allele may increase the risk of the onset of coronary heart disease. E670G PCSK.9gene polymorphism and coronary artery stenosis degree had nothing to do.4. A major risk factor for coronary heart disease include:had a history of diabetes, smoking history, reduced the level of serum HDL-C, elevated levels of LDL-C, ApoB levels, and had carried the PCSK9E670G G allele loci.5. Taking20mg of atorvastatin calcium treatment after3months, AA genotype compared with AG and GG genotype, significantly lower serum levels of LDL-C, difference had statistical significance;VLDL-C level had a tendency to reduce, but the difference had yet to reach statistical significance.(P=0.059)Conclusions:1. The Chinese Han population in tianjin region PCSK9E670G genotype and allele frequency distribution, there were differences in other parts of domestic and foreign people.2. Compared with patients with AA genotype, GG genotype carriers had a tendency to a higher level of TC, LDL, statistically significant difference (P<0.05).3. PCSK9E670G polymorphism was related to the incidence of CHD risk, G allele may increase the risk of the onset of coronary heart disease; E670G PCSK9gene polymorphism and coronary artery stenosis degree had nothing to do.4. PCSK9E670G gene polymorphism and atorvastatin calcium blood lipid regulation may have relevance in patients with coronary heart disease, after treatment AA genotype compared with AG and GG genotype, significantly lower serum levels of LDL-C, difference had statistical significance.
