ObjectiveTo observe the regulation of longbie capsule on IL-1β and TNF-α in serum of Knee Osteoarthritis (KOA) and explore whether this mechanism of effect is associated with synovial fluid-derived mesenchymal stem cells (sMSCs).MethodsAccording to the diagnostic criteria of early and mid KOA and shenxuxueyu syndrome,70patients of KOA were collected and randomly divided into treatment and control group, each group contain35cases. The treatment group received oral longbie capsules (made in the Second Affiliated Hospital of Guangzhou University of Traditional Chinese Medicine, Composition:bajitian, xianmao, tusizi, tubiechong, danshen, scorpion, centipede, long-noded pit viper, huangqi, ect.), four drugs every time, three times a day for four weeks; The control group received oral diacerein, the patients were daily given50mg after supper in the first week and given50mg after breakfast and dinner respectively one week later. Two groups of patients were taken3-5ml blood before and after drug intervention respectively. Blood was centrifuged at1500r/min in10min, and the serum was stored in freezer at-20℃. The levels of IL-1β and TNF-α were detected by Elisa method and compared before and after drug intervention between two groups. Experimental aspect:the blank, high, medium and low levels concentration of longbie capsules respectively intervene the3th generation of sMSCs in vitro culture, IL-1β and TNF-α levels in the culture medium of sMSCs were assessed and compared after the intervention between four different groups.Statistical analysis was performed using SPSS19.0software.ResuItsClinical aspects:the levels of IL-1β and TNF-α in serum have no statistically significant in two groups before treatment(P≥0.05); the levels of IL-1β and TNF-α both were dereasede in two groups, there are significant differences before and after treatment (P<0.05); IL-1βand TNF-α have statistically significant deferences in two groups after treatment(P <0.05); The differences of IL-1β and TNF-α before and after treatment were statistically significant between the two groups (P<0.05). Experimental aspects:The levels of IL-1β and TNF-α in serum showed no statistically significant between control group and blank serum groups(P≥0.05);Low concentrations group reduced IL-1β and TNF-α, but the differences had no statistically significant compared with control and blank groups (P≥0.05); Moderate and high concentration groups both can effectively reduce IL-1βand TNF-ain the culture medium of sMSCs, there were statistically significant differences compared with control and blank groups (P<0.05), but there was no statistically significant difference between the moderate and high concentration group (P≥0.05).ConclusionLongbie capsule and diacerein both can reduce IL-1β and TNF-α in serum of KOA;2. Compared to the Longbie capsules, IL-1β was decreased significantly by diacerein, However, Longbie capsules play a more obvious role in reducing the TNF-α;3. Longbie capsule can effectively reduce the level of IL-1β and TNF-α in the culture medium of sMSCs, and this role has a certain dose dependent.
