Objective:To evaluate the sensitivity and specificity of the system ELISA and the method ofcolloidal gold test strip method in detection of hepatitis B surface antigen, to explore the application value of ELISA and colloidal gold test strip method in detection of hepatitis B surface antigen.MethodsClinical Laboratory of our hospital in2012May-2013February collection of outpatient or inpatient, and physical data, a total of500cases of blood standard. Specimens were collected respectively adopt enzyme linked immunosorbent assay (ELISA), and colloidal gold test strip method (GICA), at the same time, were compared and statistically analyzed the testing results.Result1.Colloidal gold test strip method and enzyme linked immunosorbent assay in the detection of the surface antigen is closely related to viral hepatitis B, the correlation coefficient is r=0.969,P=0.000, the linear regression equation can be expressed as Log10colloidal gold test strip method=0.851Log10enzyme-linked immuno sorbent assay=0.560. 2.The difference between colloidal gold test strip method and enzyme linked immunosorbent assay in detection of hepatitis B surface antigen of Bland, through the Altman analysis method,the results show that the consistency between two detection method has95%, the results of the two methods used most of the difference values are located within the region, the results show that were detected in the hepatitis B virus surface antigen colloidal gold test strip method and enzyme linked immunosorbent assay, the results of the two methods have good consistency.3.Comparison of colloidal gold test strip method and ELISA method was used to detect thepositive rate of hepatitis B surface antigen, results are displayed in two different detection methods, the positive rate of enzyme linked immunosorbent assay of hepatitis B surface antigen was45.8%, the positive rate of strip method of hepatitis B surface antigen colloidal gold test paper is43.3%, after the chi square test, there was statistically significant difference of the positive rate between the two groups (X2=11.236, P=0.003).4. Colloidal gold test strip method and enzyme linked immunosorbent assay sensitivity method for the detection of hepatitis B surface antigen and the specificity of the two kind, sensitive method for the determination of hepatitis B surface antigen specificity and sensitivity are not all equal, then do X2partition test can be obtained by enzyme linked immunosorbent assay andthe specificity was higher than that of colloidal gold method (X2=6.056, P=0.035; X2=4.316,P=0.041).5.ROC analysis of enzyme immunoassay for detection of hepatitis B surface antigen, the ROC curve area enzyme immunoassay for detection of hepatitis B surface antigen was0.994, the standard error is0.002,95%confidence interval of0.990to0.998.6.Analysis of ROC curve of detection of hepatitis B surface antigen colloidal gold test strip method, the area of ROC curve detection of hepatitis B surface antigen colloidal gold test stripmethod is below0.778, the standard error is0.011,95%confidence interval of0.771to0.802.7.Analysis, comparison of area of ROC curve detection of hepatitis B surface antigen ELISA and colloidal gold test strip method of results, there was a statistically significant difference between ROC curve area and colloidal gold test strip method for the detection of hepatitis Bsurface antigen ROC curve ELISA in detecting hepatitis hepatitis B surface antigen under (P=0.026).ConclusionEnzyme linked immunosorbent assay (ELISA) method is used to detect the positive rate,sensitivity and specificity of HBsAg are higher than that of colloidal gold method which is the main method to the detection of HBsAg. Colloidal gold method and ELISA results show a close correlation and difference is small. Colloidal gold is easy to carry,the method is more simple and intuitive, the result is more efficient and does not require special equipment, more suitable for infection screening, emergency patient testing and so on.
