Analysis On The Rare Patterns And Weakly Positive Results Of Hepatitis B Serologic Testing By ELISA And The Strategy Of Interpreting Commenting

Objective: To detect quantitative results in hepatitis B surface antigen(HBs Ag)weakly positive,both HBs Ag and hepatitis B surface antibody(HBs Ab)positive,both anti-HBe antibody(HBe Ab)and anti-hepatitis B core antibody(HBc Ab)positive or one of them positive,HBs Ab positive and both HBe Ab and HBc Ab positive or one of them positive modes in 5 items of hepatitis B detected by enzyme-linked immunosorbent assay(ELISA)and analyze their positive rates of HBs Ag,thereby providing the gist of interpreting commenting.Methods: 56 cases of HBs Ag weakly positive,58 cases of both HBs Ag and HBs Ab positive,80 cases of both HBe Ab and HBc Ab positive or one of them positive and 93 cases of HBs Ab positive and both HBe Ab and HBc Ab positive or one of them positive modes,whose quantitative values were re-detected by chemiluminescence immunoassay(CLIA),were screened out.The positive rates of HBs Ag in different groups were also compared.Results: The positive rates of HBs Ag in group 1±2-3-4-5-and 1±2-3-4+5+ re-detected by CLIA were 15.4% and 100%(P<0.01),respectively;the positive rates of HBs Ag in group 1+2+3-4-5-and 1+2+3-4+5+ re-detected by CLIA were 5.0% and 93.8%(P<0.001),respectively,and the positive rates of HBs Ab in the two groups were 100.0% and 43.8%(P < 0.001),respectively,and the probabilities of both HBs Ag and HBs Ab being positive in the two groups were 5.0% and 37.5%,respectively(P<0.01);the positive rates of HBs Ag in group 1-2-3-4-5+ and 1-2-3-4+5+ re-detected by CLIA were 10.8% and 35.7%,respectively(P<0.0167).The difference between the above two groups was statistically significant.HBs Ag in HBs Ab positive and both HBe Ab and HBc Ab positive or one of them positive mode were all negative after re-detection by CLIA.Conclusions: The results of HBs Ab,HBe Ab and HBc Ab,and the ELISA patterns have important guiding significance for judging HBs Ag results.HBe Ab and HBc Ab both being negative suggests the possibility of false positivity of HBs Ag in HBs Ag weakly positive,both HBs Ag and HBs Ab positive ELISA modes.HBc Ab positive alone,both HBe Ab and HBc Ab positive ELISA modes suggest the possibility of false negativity of HBs Ag.The possibility of false negativity of HBs Ag in HBs Ab positive and both HBe Ab and HBc Ab positive or one of them positive ELISA mode is lower.Re-detection advice of HBs Ag should be given according to the levels of HBe Ab and HBc Ab.
