The Relationship Between Vertebral Artery Dominance And Vertigo And Basilar Artery Curvature

Objective:In order to investigate the relationship of vertebral artery dominance(VAD)with vertigo and basilar artery curvature, analyzing the mechanisms of vertigo thatcaused by VAD.Method: A total of183patients with vertigo patients as the chief complains wereenrolled. They were divided into VAD group and non-VAD groups according to theresults of MRA. The severity of vertigo, the incidence of BA curvature、direction、theshape of BA and BL in two groups were compared. We analyzed the relationshipbetween VAD and vertigo and the correlation between diameter difference of bilateralvertebral artery and BL in VAD and BA curvature group. SPSS17.0statistical softwarewas used for data processing. Continuous variables were expressed as mean±SE.Results:（1）Between vertebral artery dominance group and non-vertebral arterydominance group，differences of sexuality，age，hypertension，diabetes，smokinghistory，history of alcohol，cervical spondylosis and risk factors were not statisticallysignificant（P＞0.05）.（2）There was statistically difference in the severity of vertigo between the severevertebral artery dominant group and the non-VAD group(3.31±1.28versus2.54±1.46)(P=0.000).The treatment time of VAD group was higher than non-VADgroup(4.52±1.77versus3.28±1.45，P=0.008）. （3）Of98VAD patients，70.41%patients dominant VA were left higher than that29.59%patients were right.（4）Compared with that of non-VAD group, the incidence of BA curvature with VADwas significantly higher[54.1%(53/98) versus16.5%(14/85),2=27.75，P=0.000].InVAD group，12patients were bend to the left,41patients were bend to the right.Most ofthe curvature were C-bend.（5）The BL of VAD group was significantly higher than that in non-VAD group. Nodifferences were found in basilar artery diameter，BAL between two groups(P＞0.05).（6）Diameter difference of bilateral VA with BL and the severity of vertigo，showedpositive correlation,and there was statistical significance (P=0.000).Conclusion:1.Patients with vertigo has higher incidence of VAD. Compared withnon-VAD group, the incidence of abnormal BA shapes，the severity of vertigo andtreatment time were higher in VAD patients.2.In VAD patients the incident of BA curvature was higher than the non-VAD group andBA usually bends to the non-dominant side. Basilar artery curvature were mostlyC-bend in two groups.3.Diameter difference of bilateral vertebral artery and BL was positively correlated,The process of VAD may increase BL and the severity of vertigo.
