Abstract/Summary:
Background Cerebrovascular disease in the elderly has become a hazard to our health and lives. Atherosclerotic plaque is the main reason for ischemic stroke. Currently, atherosclerotic plaque lipid composition according to the classification can not meet the needs of clinical, which requires us to plaque deeper, more detailed analysis. We mainly through the study of the atherosclerotic plaque surface morphology to analyze the relationship between different forms of surface patches with different risk factors and sonographic between treatment individualized treatment clinic.Purpose Classifying the carotid plaque surfaces of ischemic cerebrovascular disease patients by color Doppler flowing imaging, and analyze the correlation between various risk factors and different types of carotid plaque surfaces(unstable plaque), and make it a guidence to clinical treatment.Method We selected inpatients of ischemic cerebrovascular of our department from January to July in2013, there were359all, at last260were kept to be analyzed. Basic conditions were written down carfully such as statistics blood pressure, blood sugar, blood lipids, smoking data. And clinical data were demanded such as cranial MRI+MRA+DWI, neck and other related intracranial vascular ultrasound examination of blood vessels according to the results of ultrasound for all patients. Data is in method of mean standard deviation. In an exploratory analysis, we assessed the association between the presence of dense plaques and possible determinants that risk factors for cerebral vascular stenosis and the severity (smoking, hypertension, hypercholesterolemia, diabetes, previous cerebrovascular disease). All factors are incorporated into the multivariate logistic regression model to assess the correlation between the complexity of plaques detached from other factors. And forward method using a Wald test. Expressed as OR and95%confidence intervals, which mean that the use of P<0.05was examined statistically significant value. Repeat the analysis, only ulcers spot. All calculations were performed using SPSS16.0for Windows.Result Logistic regression analysis about risk factors of cerebrovascular disease in irregular patches and ulcerated plaques showed that the results of hypertension (OR=0.443,95CI:0.214-0.919, P<0.05), diabetes (OR=2.628,95CI:1.178-5.866, P<0.05), hyperlipidemia (OR=0.427,95CI:0.192-0.953, P<0.05) was ulcerated plaque block independent risk factor, equally, hyperlipidemia (OR=0.469,95CI:0.231-0.953, P<0.05) were independent risk factors for irregular patches, after adjusting for other risk factors as age, gender, etc.Relationship of plaque surface morphology with echo of plaques, X2=13.919P=0.008, showed that there is correlation between the types of plaque morphology classification of the ultrasonic echo.Conclusion1. Atherosclerotic plaque morphology classification shows that there are differences in risk factors for different types of plaque formation surface, high blood pressure, diabetes is associated with the formation of plaque ulceration relevant, high-fat hyperlipidemia is ulcerated plaques and irregular plaque common risk factors;2. Depending on the morphology of plaques, targeted intervention risks factors, to the clinical treatment of individuals.
