Backgrounds and ObjectivePrimary hepatocellular carcinoma is one of the most common malignant tumors in our country, including hepatocellular carcinoma accounted for more than90%.It growths rapidly, and is easy to metastasize,which leads to poor prognosis. Therefore, it is particularly important to further clarify the pathogenesis of HCC and look for the measures of early diagnosis, early treatment, and control transfer.Renin-angiotensin system a important hormonal system in human body. it is an important role in maintaining the stabilization of blood pressure, the extracellular fluid volume balance and normal function of local tissue and organ. In recent years we find that there is local RAS in multiple tissues and organs such as brain, kidney, lung, pancreas, liver. Studies showed that the classic intrahepatic RAS shaft (ACE-AngⅡ-ATIR) participates in the growth, apoptosis, inflammation, and fibrosis process of organization cell, and possibly plays a vital role in the development of turnor.ACE2is the only ACE isozyme that has been found currently. Its emergence resusts in the great changes of the classic RAS, which started the study of the new branches of the RAS:ACE2-Ang (1-7)-Mas receptor axis. Some reports have confirmed that ACE2has the effect of inhibiting tumor development in pancreatic cancer, endometrial cancer and so on, but at the moment, the relationship between ACE2and HCC and clinical significance were not reported at home and abroad.The expression of ACE2and AT1R in HCC, liver cirrhosis and normal tissues near the cancer were detected by immunohistochemical method in this study.The aim is to explore the expression of ACE2and AT1R in HCC tissues, their relationship and the significance on the pathogenesis of HCC. It is expected to provide certain theoretical basis for HCC in clinical diagnosis and prognosis judgment,and provide the new mentality for the research of treatment.Methods82hepatocellular carcinoma samples,40pericarcinomatous cirrhosis specimens and24normal liver tissue specimens were Collected by operation from June2011to June2012randomly in the first affiliated hospital of ZhengZhou university. Immunohistochemistry was used to detect the expression of ACE2and AT1R in these tissue. The relevance of expression of ACE2and AT1R in hepatocellular carcinoma was analysed and the correlation of the expression and clinical pathological features was statistics.We used SPSS17.0software to analyze database, and used variability chi-square test to analyze the express of ACE2and AT1R in the three tissue, and then used spearman correlation analysis to analyze the relation between ACE2and AT1R in HCC and used a=0.05as inspection level.ResultsThe positive expression rates of AT1R in carcinoma, cirrhosis and normal tissue specimens were separately86.6%,35.0%and4.2%, and significant difference was subsistent between each other(P<0.016). The positive expression rates of ACE2in carcinoma, cirrhosis and normal tissue specimens were separately81.7%,77.5%,8.3%, and the difference in them was significant(P<0.05), however, there was no statistical significance of the difference between carcinoma and cirrhosis(P>0.016). There was a positive correlation between the expression of ACE2and AT1R in cancerous tissues (P<0.05). In addition, the expression of ACE2and ATIR was closely related to tumor differentiation, clinical stage and invasion in Portal vein. but had no obvious correlation with the patient’s gender, age and tumor size.Conclusion1. ACE2and AT1R are expressed highly in HCC, and the expression is closely related to the differentiation and invasiveness of HCC.2. Combined detection of ACE2and AT1R protein in HCC may have certain value for early diagnosis and prognosis judgment.3. ACE2and AT1R participate in the occurrence and development of HCC. That further clarify its mechanism is helpful to the research of prevention and treatment of HCC.
