Objectives:Through the investigation of the situation of pregnancy care andpregnancy outcomes, to understand他the incidence of adverse pregnancyoutcomes,and explore the effect of pregnancy care on pregnancy outcomes.Provided favorable data support and scientific basis for improving theservice of pregnancy care,and reducing the adverse pregnancy outcomes.Method:Cross-sectional study are adopted in this investigation.During July1,2012to December31,2012,randomly taken some puerpera who delivered inMCH Hospital of five city and five county in hunan province as theresearch object. The investigation content: the general situation ofpuerpera and her husband and their family, and the general situation ofpregnancy care and pregnancy outcomes. A total of1000questionaires,recycled995, effective questionnaires was958, questionnaires effectiverate was95.8%. Used Epidata3.1establish database, and took spss19.0statistical software for statistical analysis. Adopted the descriptivestatistical analysis to analysis the general situation of puerpera andpregnancy outcomes,and adopted chi-square test to take single factoranalysis of hypertensive disorder complicating pregnancy, gestationaldiabetes mellitus, premature birth, pathological pregnancy andpostpartum hemorrhage. Then the selected statistically significantfactors would be taken Logistic regression analysis, using a=0.05as thetest standard. Result:1.In this investigation object,the incidence of hypertensivedisorder complicating pregnancy was12.2%, gestational diabetes mellituswas3.7%, premature birth was8.4%, pathological pregnancy was2.3%,postpartum hemorrhage was9.4%.2. The single factor analysis showed that Maternal age, CouplesEducation, household income, preconception care consultation, pregnancymedical examination, prepregnancy smoking and drinking, exposure totoxic and harmful substances, mother pregnancy frequent contactwith animal, drugs, maternal prepregnancy obesity, under stress, sufferssickness, whether vaccination, whether folic acid was statisticallysignificant to the adverse pregnancy outcomes(p<0.05).3. The multi-factor Logistic regression analysis showed thatvenerable age(OR=1.435), reproductive history(OR=1.938), medicationuse(OR=2.576), unintended pregnancy(OR=4.672) were the risk factorof hypertensive disorder complicating pregnancy;mother pregnancy cantake the initiative to avoid passive smoking(OR=0.314) is the protectivefactor of hypertensive disorder complicating pregnancy; gravida in thecity is more susceptible to hypertensive disorder complicating pregnancythan in rural areas(OR=0.580); gravida progestation smoking(OR=2.338),drinking(OR=2.338)and suffering sickness(OR=2.761), reproductivehistory (OR=2.379)is a risk factor of gestational diabetes mellitus;initiative to avoid passive smoking(OR=0.384) were the protectivefactor;husband progestation smoking (OR=1.616), gravida frequentcontact with animal(OR=2.030) were the risk factor of premature birth;gravida weight is too low(OR=0.598) and high income families (OR=0.642)were the protective factor;gravida weight is too high(OR=2.617),suffering sickness(OR=4.379), gravida progestation smoking(OR=6.667),frequent contact with animal(OR=6.752),husband progestation drinking (OR=5.456)were the risk factor of pathological pregnancy induced labor,gravida highly educated(OR=0.385)and folic acid (OR=0.102)were theprotective factor. Gravida have medical checkups the first6month ofprogestation(OR=0.634)and avoid passive smoking (OR=0.464)were theprotective factor of postpartum hemorrhage, gravida progestation havedisease(OR=1.701)reproductive history(OR=2.287)were the risk factor.Conclusion:1. The incidence of hypertensive disorder complicating pregnancy was12.2%, gestational diabetes mellitus was3.7%, premature birth was8.4%,pathological pregnancy was2.3%, postpartum hemorrhage was9.4%.2.The main influence of the adverse pregnancy outcomes are Maternalage, Couples Education, household income, preconception careconsultation, pregnancy medical examination, prepregnancy smoking anddrinking, exposure to toxic and harmful substances, motherpregnancy frequent contact with animal, drugs, maternal prepregnancyobesity, under stress, suffers sickness.
