Objective:To explore the clinical significance and the correlation of iron metabolism indexes and thrombelastography parameters in patients with type2diabetes mellitus.Methods:To select from February2013to March2014in Haikou City People’s Hospital Department of endocrinology in out-patient and ward for treatment of patients with type2diabetes mellitus (T2DM)145cases while selecting the normal control group (Normal group)80cases; According to the presence or absence of coronary heart disease (CAD), the patients with type2diabetes mellitus divided into simple type2diabetes mellitus group (simple T2DM group, n=105cases) and type2diabetes mellitus complicated with coronary heart disease group (T2DM with CAD group, n=40cases); According to control target of diabetes in internal book (the sixth edition), the simple T2DM patients were divided into group A:curative effect good group (HbAlc<7.5%, n=48cases) and group B:poor response group (HbAlc>7.5%, n=57cases); Assessed for SI, SF, TRF, HbAlc, FPG and FINS with Chemiluminescence and biochemical assessment. Utilized TEG5000coagulation analyzer for the determination of R value, K value, MA value, CI value and the alpha angle; To compare the Normal group and T2DM group with age, BIM, SBP and DBP; To compare the Normal group and the simple T2DM group with SI, SF, TRF, HbAlc, FPG and FINS; To compare the A group and B group with SI, SF, TRF, R value, K value, MA value, CI value and the alpha angle; To compare the Normal group, simple T2DM group and T2DM with CAD group with R value, K value, MA value, CI value and the alpha angle;To analyze the correlation of each indicator and SF in patients with simple T2DM. Also, multivariate stepwise regression analysis was used to denstify the factors of SF.Result:1. Compared with Normal group, SBP, BMI in T2DM group significantly increased (p<0.05);2. Compared with Normal group, FPG, HbAlc, FINS and SF in simple T2DM group significantly increased (p<0.05);3. Compared with group A, SF in group B significantly increased (P<0.05);4. Compared with Normal group, R value, K value in T2DM with CAD group significantly lowed (p<0.05), whereas alpha angle was increased (p<0.05); the MA value and CI value were higher (p<0.05); Compared with simple T2DM group, R value, K value in T2DM with CAD group significantly lowed (p<0.05), whereas alpha angle was increased (p<0.05); the MA value and CI value were higher (p<0.05);5. Correlation analysis of SF and the indicators in simple T2DM group:FGP was positively correlated with SF (r=0.340, p=0.003), HbAlc was positively correlated with SF (r=0.336, p=0.004), R value was positively correlated with SF (r=0.323, p=0.005);6. Multiple regression analysis of the factors affecting the level of SF in simple T2MD group:the SF as the dependent variable, HbAlc, FPG, INS, R value, K value, MA value, CI value and the alpha angle as variables, multivariate stepwise regression analysis showed:FPG and R values are the factors of SF, the standard regression coefficient b were0.348,0.031; the P values were0.004,0.005; the regression equation was SF=-326.176+26.53FPG+59.71R.Conclusion:The patients with Type2diabetes mellitus iron metabolism disorder and serum ferritin significantly increased; In patients with type2diabetes mellitus, the more poor treatment effect, serum ferritin increased significantly; TEG parameters did not change significantly in patients with simple type2diabetes mellitus and no significant changes in curative effect good group and poor response group, suggesting that coagulation did not show significant changes in patients with simple type2diabetes mellitus, however type2diabetes mellitus complicated with coronary heart disease group showed a hypercoagulable state; There is a certain correlation between type2diabetes patients with serum ferritin, sugar metabolism and coagulation function; FPG and R values are the factors of SF. Can be better monitored by joint detection TEG parameters of iron metabolism indices and efficacy in patients with type2diabetes, and individualized targeted treatment, prevention and treatment of type2diabetes mellitus due to cardiovascular disease and coagulation abnormalities caused.
