part1A study of constipation of community children aged0to4years oldObjective: To survey constipation of community children aged0to4years.Methods: Children aged0to4years(who were born in January,2010to December,2013) lived in Daxigou community health service centers(survey site) in Yuzhong District of Chongqing were enrolled into the survey.A questionnaire and telephone inquiries were investigated to get stool statusof children from October to December2013. Parents reported stool stypes oftheir children according to the figure of Bristol Stool Form Scale,thediagnosis of constipation were obtained according to Rome Ⅲ criteria.Sensitivity, specificity, positive likelihood ratio and negative likelihoodratio were calculated combined Bristol type1to3with major items of thediagnostic criteria.Results: There are1206children in total under4years old in thecommunity,1050questionnaires were gave out,1000questionnaires were retrieved,972questionnaires were valid.87.1%(1050/1206) of childrenparticipated in our survey, the rate of effective participation was97.2%(972/1000).120(12.3%,120/972) children with stool type1-3were reportedby their parents according to the figure of Bristol Stool Form Scale,61.5%(598/972) of the children whose type of defecation were stool type4,the children with stool type5-7were26.1%. The type of defecation of55children was stool type1-3and all of them suffered from constipation in thispaper. The prevalence rate of constipation for children was5.6%(55/972),boys and girls were affected with equal frequency (X squared=2,p=0.157>0.05).13(5.4%,13/240) infants,23(6.1%,23/374) toddlersand19(10.8%,19/205) preschool children had constipation. Sensitivity andspecificity of the Rome III criteria of defecation less than twice per week andhistory of defecation pain or difficult combined with Bristol type1to3were100%and over96.8%, respectively and positive likelihood ratio of theformer was higher than that of the latter (142.9vs31.3).Conclusion: The defecation of infancy was Bristol type6in themajority, and that had transitted to Bristol type4with the increasing of age.The incidence of constipation was5.6%in children under4years old. Therewere no significant gender differences. The stool type of children was relatedto dietary. Children with Bristol type1-3had high risk for constipation. part2Survey of dietary fiber for intake in childrenObjective: To study intake of dietary fiber in constipated children.Methods: The constipated ch ildren aged6months to4years wereselected randomly as research objects, weighing method of72hours wereadopted to survey children’s dietary,and normal children were controlgroup.After symptoms improved, the constipated children of follow-upaccepted dietary survey again with the same method,and compared tothemselves. Nutritional status were analysed by nutrition software.Results:70children aged6months to4years were enrolled into thesurvey,50children’s parents agreed to dietary investigation,22children’sparents completed secondary dietary survey. And another35normal childrenaged6months to4years were participation in dietary survey.The intake ofdietary fiber in infancy with constipation was1.2g/d, which was lower thanthe normal infant whose was4.0g/d. The intake of dietary fiber ofconstipated children aged1to4years were3.9g/d, and the normal childrenwas5.9g/d. The intake of dietary fiber was increased significantly in all22relieved children according to the second analyzsis of diatery.Conclusion: The intake of dietary fiber in children with constipationwas lower than normal children, Symptom of constipation can be improvedby increasing fiber.Most of the normal children’s dietary fiber intake arelower the recommendation criteria. such as AHF, FDA, NAS, recommendation of diatery fiber for children need further studying.
