Aclinical Effectiveness Analysis Of Femoral Neck Fracture Fixed With7.5mm QWIX Screws

Purpose: To evaluate the clinical effectiveness of the femoral neckfractures fixed with7.5mm QWIX screws, and to find out the associatedhigh-risky factors which would lead to avascular necrosis of femoral headoperatively.Methods: A retrospective study for2years was performed on53patients with femoral neck fracture at the age of14to83, who had theoperations with QWIX screws to fix the femoral fracture between January2009and March2013in the first affiliated hospital of Chongqing MedicalUniversity. The average patient age was45.7years old (range14to83years). We investigated the Harris scores, the healing process of fractures,the situation of fixation, and the AVN ratio of femoral head. Also werecorded the associated risky factors include patients’ age, gender, traumaenergy, wound site, associated injuries, interval of operation, Gardenclassification, classification of fractures by anatomic sites, Garden index,operation lasting time, comminuted bones on rear side, and the type ofoperations. And all these records wound analysis by Pearson chi-square testand binary Logistic regression analysis of multi-factors.Results: The mean Harris scores was91.2(68to100points) points. And of those cases,42were excellent,6were good,1was fair,4were bad.There was no non-union happened during the follow-up period. All theinternal-fixations were fixed firmly, there was no fixation loose or cut-out,and only4cases involved with femoral head avascular necrosis whichoccurred at12to15months operatively, the ratio of AVN was7.5%. Wefound out that the femoral head avascular necrosis has marked relationshipswith the type of fractures and the way of operations performed.Conclusions: We found out that the clinical effectiveness of thefemoral neck fractures was excellent when7.5mm QWIX screws were used.And we concluded that the type of femoral neck fracture was associatedwith the results of operations, especially the Garden classification whichindicated the displacement of the fractures was important to predict theAVN ratio of femoral head. And we found that the AVN ratio of femoralhead increased when open reduction was performed.
