Object:To explore the predictive factors of the poor outcome of cerebral venous sinus thrombosis (CVST). Methods:The clinical data of42patients with CVST were analyzed retrospectively, after hospitalization, the clinical outcome was assessed with the modified Rankin Scale(mRS) when they out of the hospital. And then divided these patients into good outcome group and poor outcome group.Comparing the demographic、 etiological、clinical features and other related factors between the two groups,multivariate Logistic regression analysis was used to determine the independent predictors of the poor outcome.Results:The mRS scores for42patients with CVST:0in3cases,l in10cases,2in16cases,3to5in11cases,and6in2cases.31%of patients had poor outcome (mRS score>2),and the mortality rate was4.8%.Univariate analysis showed that the infection of central nervous system、cancer pregnancy、puerperium、oral contraceptive or hormone replacement therapy、hyperhomocysteinemia、therapy and the level of D-dimer were the possible risk factors for outcome. Multivari ate Logistic regression analysis and the result of the ROC curve showed that D-dimer greater than990ng/ml was an independent predictor of poor outcome in patients with CVST(odds ratio,1.006,95%confidence interval1.002to1.011,P=0.005) in hospital; The anticoagulation with thrombolysis therapy was the most effective to the patients with CVST(odds ratio,0.027,95%confidence interval0.002-0.447,P=0.033).Conclusion:D-dimer greater than990ng/ml was an independent predictor of poor outcome in patients with CVST(odds ratio,1.006,95%confidence interval1.002to1.011,P=0.005) and carrying out the anticoagulation with thrombolysis therapy was the most effective to the patients with CVST in hospital.
