Objective:To explore the consistency between preoperative clinical TNM stage and postoperative pathology TNM stage of gastric cancer and look for the under reason. Methods:145patients of gastric cancer with complete clinical data from March2012to December2013in Xinjiang medical university affiliated tumor hospital were enrolled into this study. All cases were performed postoperative pathologic TNM stage according to the TNM stage criterion of gastric cancer from IUCC(the7th edition,2010),and the results were compared with preoperative TNM stage, then we studied the coincidence rate of both staging by analysis of the consistency of T staging, N staging and M staging using SPSS18.0.Results:Compared Preoperative clinical T1, T2, T3, T4staging and the postoperative pathologic staging Kappa value was.0.545,0.410,0.561,0.519; Compared preoperative T staging and the postoperative pathologic staging Kappa value was0.506; Compared EUS staging with CT staging on T1phase, postoperative pathologic staging Kappa value were0.492and0.643, on T2stage were0.541and0.558; Compared combination of CT and EUS preoperative T1staging and the postoperative pathologic staging Kappa value was0.704; Compared preoperative clinical N staging and the postoperative pathologic staging Kappa value was0.391; Compared preoperative overall staging and the postoperative pathologic staging Kappa value was0.433as a whole. Conclusions:According to the new TNM stages, the author can make a accurate TNM stage and individual treatment plan for gastric cancer patients in clinic. But the overestimate and underestimate still existed, especially in N staging. Therefore, make a accurate judge for N staging according to all kinds of preoperative clinical data and images is the key for improvement of preoperative TNM staging accuracy and treatment.
