The Effect Of Pulmonary Rehabilitation Procedure In Patients With Acute Exacerbations Of Chronic Obstructive Pulmonary Disease

Objective:To investigate the effect of pulmonary rehabilitation procedure on patients with acute exacerbations of chronic obstructive pulmonary disease.Method:38patients with an acute exacerbation of chronic obstructive pulmonary disease (AECOPD) were enrolled in this study and randomized into rehabilitation group (n=20) or the control group (n=18). The control group was received oxygen therapy, antibiotics, bronchodilators, hormone therapy, at the same time, given the breath control guidance and daily physical activity guidance; on the basis of the control group, rehabilitation group was given a7days three-stage rehabilitation intervention, which included breath control, active cycle of breathing technique, resistance training, gait training. Exercise capacity evaluation (30-second sit-to-stand test and30-second arm curl test), quality of life (Modified Barthel Index and Self Assessment of Chronic Obstructive Pulmonary Disease, CAT), mental state (Hospital Anxiety and Depression Scale, HAD) were evaluated both before and after the intervention. The white blood cells and high-sensitivity C-reactive protein was detected to evaluate the safety of this procedure.Two months after discharge, a telephone follow-up was taken to compared the level of self-reported daily physical activity between both groups.Result:After the period of7days of intervention, compared with the control group, the number of30-second sit-to-stand test(16±2.18vs13.33±2.28,p<0.05) and30-second arm curl test(19.40±1.76vs17.44±1.69, p<0.05), the score of Modified Barthel Index (93.30±6.09vs83.28±11.95,p<0.01), and the level of the self-reported daily physical activity were significantly higher in rehabilitation group; the score of CAT(14.55±3.09vs18.67±3.41, p<0.01)and anxiety(5.90±2.65vs7.56±1.85,p<0.05) has decreased significantly in rehabilitation group; no significant difference of white blood cells and high-sensitivity C-reactive protein between both groups,p>0.05.Conclusion:(1) The7days three-stage pulmonary rehabilitation procedure can improve the exercise capacity_and the quality of life of patients with AECOPD, alleviate their anxiety, and improve daily physical activity within2months after discharge from hospital.(2) The acute rehabilitation procedure is safe and feasible, the rehabilitation group patients in the rehabilitation procedure were well tolerated, without aggravating systemic inflammation after the intervention.
