Epidemiological Characteristics And Related Factors On The Four Kinds Of Infectious Diseases Among The Entry-Exit Personnel At Hunan Port

Objective:This study is aimed to analyze the status, the populations and the epidemiology of four kinds of infectious diseases(AIDS, syphilis, hepatitis B, hepatitis C) and find out the related factors of the infectious diseases at Hunan port of Entry-exit Personnel in Hunan Province from2007to2012, providing a scientific basis for preventing and surveillance of infectious disease and health interventions at port.Methods:In this study, we selected Entry-exit Personnel received healthy check-up in Hunan international travel health center during January1,2007to December31,2012as the research object. The object monitoring serum samples was collected for HIV antibodies (ELISA screening method, protein blotting confirmed), syphilis (TRUST Act screening, TPPA recognized), hepatitis B surface antigen (ELISA method) testing, hepatitis C antibody (ELISA method) testing, and physical examination and serological tests for infectious diseases. Epidemiological case investigation would be carried out if found positive. The content of the research mainly included demographic characteristics epidemiological characteristics and infection status of four kinds of major infectious diseases (HIV, syphilis, hepatitis B, hepatitis C) of Entry-exit Personnel in Hunan Province.Results:A total number of72,316received healthy check-up at Hunan port from2007to2012. Male accounted for79.7%, and the male to female ratio was3.9:1. Their age were mainly30-49years old, accounting for75.3%.63,722people were Chinese nationals personnel, accounting for88.1%.34,755people were laborers, accounting for48.1%. Number of Entry-exit Personnel showed an increasing trend from2007to2012. For the total number of72,316people,5,266cases were diagnosed as infectious diseases, and hepatitis B surface antigen positive was the most, accounting for82.9%, followed by treponema pallidum antibody positive (9.0%), Hepatitis C antibody positive (7.5%), HIV antibody positive (0.6%). For the total number of72,316people,4,364cases were diagnosed as HBsAg positive, infection rate was6.03%;396cases were diagnosed as HCV positive, infection rate was0.55%;473cases were diagnosed as syphilis positive, infection rate was0.65%;33cases were diagnosed as HIV positive, infection rate was0.05%.A total number of5266cases of four types of infectious diseases, the overall infection rate was7.28%. Male suffered a higher HbsAg prevalence than female, the difference was statistically significant (P0.001); female suffered a higher HCV prevalence than male, the difference was statistically significant (P<0.001); while syphilis and HIV infection in terms of gender difference was not statistically significant (P>0.05). People in40-49years suffered a higher syphilis prevalence than other years group, and HCV, syphilis and HIV infection had a statistically significant (P<0.001) differences at age. Foreign marriage, laborers and business people suffered a higher HbsAg prevalence than other people. HbsAg, syphilis and HIV infection had a statistically significant (P<0.001) differences at personnel category. Chinese nationality suffered a higher syphilis, HbsAg prevalence than foreign nationality immigration officers, the difference was statistically significant (P<0.001); foreign nationality suffered a higher HIV prevalence than Chinese nationality, the difference was statistically significant (P<0.001). HIV, HbsAg and syphilis infection were significant difference between years (P<0.05). Multivariate analysis showed that males (OR=2.512,95%CI:1.758-3.552), personnel category of foreign marriage (OR=1.507,95%CI:1.430-1.589), travel to visit relatives (OR=1.502,95%CI:1.405-1.708), laborers (OR=1.503,95%CI:1.426-1.584) and crew (OR=1.205,95%CI:1.160-1.252), and low age (OR=1.479,95%CI:1.402-1.559) were the influencing factors of4kinds of infectious diseases at Hunan port.Conclusion:The number of Entry-exit Personnel at Hunan port showed a trend of increase from2007to2012. Most of them are male, young and middle-aged workers. Most of the infectious cases are hepatitis B surface antigen positivet, whose infection rate is6.03%; and the HCV infection rate was0.55%; the syphilis infection rate is0.65%; HIV infection rate is0.05%. A total number of5266cases of four types of infectious diseases, the overall infection rate was7.28%. Males, personnel category of foreign marriage, travel to visit relatives, laborers and crew (OR=1.205,95%CI:1.160-1.252), and low agewere the influencing factors of4kinds of infectious diseases at Hunan port.
