ObjectiveTo discuss the effect of the hemoglobin concentration in the bloodwith the symptomatic cerebral vasospasm occurred in aneurysmalsubarachnoid hemorrhage patients after surgical treatment.MethodsA retrospective analysis of218cases of surgical treatment of thepatient’s medical record information after aneurysm ruptured, Includinggeneral data, surgical procedures,whether intraoperative aneurysm ruptured,Hunt-hess grading on admission, etc. the initial CT result after aneurysmruptured is improved Fisher grading, detailed records the mean hemoglobinconcentration of each patients admitted to hospital after after surgicaltreatment. Patients with postoperative mean hemoglobin concentration canbe divided into below110g/L,110-120g/L,120-130g/L,130g/L of abovefour grades. First to single factor analysis of possible factor effectedSymptomatic cerebral vasospasm, then the factor which was statisticsignificant in single factors analysis is in multiple factors stepwise logistic regression analysis.Results1The single factor analysis reveal that Hunt-hess grading onadmission, whether intraoperative aneurysm rupture,CTFisher grading,postoperative mean hemoglobin concentration was associated withsymptomatic cerebral vasospasm significantly in aneurysmal subarachnoidhemorrhage patients after surgical treatment(P<0.05).2After adjustment the confounding factors of Hunt-hess gradingon admission and CTfisher grading,the multi-factor analysis reveal thatpostoperative mean hemoglobin concentration was associated withsymptomatic cerebral vasospasm occurred significantly,compared withother levels,Patients whose postoperative mean hemoglobin concentrationis in110-120g/L have the lowest incidence of symptomatic cerebralvasospasm (OR=3.293,3.633,5.336,P<0.05).ConclusionHemoglobin concentration is an independent risk factor,which causessymptomatic cerebral vasospasm occurred in patients with aneurysmalsubarachnoid hemorrhage after surgical treatment,Keeping hemoglobinconcentration at a certain level to reduce the occurrence of symptomaticcerebral vasospasm of aneurysmal subarachnoid hemorrhage patiets aftersurgical treatment.
