| Objective:To evaluate and compare clinical-outcomes of percutaneous vertebroplasty and percutaneous kyphoplasty for osteoporotic vertebral compression fracture nonunion, discuss the best surgical treatment of osteoporotic vertebral compression fracture nonunion.Method:According to X-rays, CT and MRI imaging data and clinical specialist examination, osteoporotic vertebral compression fractures nonunion in elderly female patients were admitted to hospital. The patients who were accompanied by spinal cord compression or nerve root compression symptoms, selected open surgery and were not recorded into the group, since PVP or PKP treatment may aggravate the symptoms of spinal cord compression or nerve root, for what was the use of bone cement relative contraindications; the other part of patients who were not associated with spinal cord compression or nerve root symptoms, chose two minimally invasive surgical approaches(including PVP or PKP). Finally we select48cases of application PVP or PKP treatment, the follow-up of whom achieved good。Comparison of visual analogue scale (VAS) score, Oswestry Disability Index (ODI) score, anterior, middle vertebral body height, kyphosis angle, the amount of bone cement filling and leakage rate of2groups before and after surgery.Results:Group A and B of age, preoperative VAS score, ODI score and thoracolumbar kyphosis angle was no significant difference. Subjective satisfaction with group B showed no significant difference. Postoperative VAS score and ODI score of two groups compared with preoperative VAS score and ODI score significantly reduced, but two group showed no significant difference. Vertebra reduction of group B was more effective than group A; Group B of vertebral kyphosis was better than group A. Group A of13vertebra bone cement leakage (54.17%) and Group B of6vertebral bone cement leakage (25%), Group A of bone cement leakage rate was higher than group B.Conclusion:Minimally invasive surgical approach on patients of osteoporotic vertebral compression fractures nonunion of PVP compared with PKP, both of them can relieve back pain; the former not only shorten the operation time, reduce the operation wound, but also can reduce the economic burden of the patients, but the bone cement leakage rate is more than PKP, the latter provide a better vertebra reduction. |
