Objective:Large cell neuroendocrine carcinoma of the lung is a newly recognized clinicopathologic entity. The clinical characteristics and optimal treatment of patients with large cell neuroendocrine carcinomas are not yet established. The aim of this study was to analyze the clinical characteristics, therapeutic modalities and survival of large cell neuroendocrine carcinoma(LCNEC), providing the basis for further improve its diagnosis and treatment level. Methods:We retrospectively examined the chart of total of33patients with the LCNEC in our hospital from2007.1to2013.11, and followed up its therapeutic modalities and prognosis. By the use of SPSS17.0statistical software for statistical analysis,Kaplan-Meier method for calculating survival rate,log-rank non parametric test for univariate analysis, and P-values less than0.05were considered to be represent statistically significant difference. Results:The ratio of males to females was5.6:1in33LCNEC patients. The oldest patient was80years, the smallest was37years,the median age was56.2years. The proportion of smokers was73.3%, First symptoms were cough, dyspnea, sputum, hemoptysis, chest pain and swollen lymph nodes, and physical findings admitted; Only5patients showed pathologic stage I, The other stages were Ⅱ(11),Ⅲ(12), and IV(5).Lobectomies was performed in19patients, pueumonectomies in6, and except1explorative thoracotomy, all of them had lymph node dissections. No perioperative death occurred. Preoperative diagnosis rate was11.5%,Follow-up to November2013.The range of survival time was from3months to44months. The median survival time survival rates were29months, The1-,2-and3-year survival rates were68.3%,29.7%,6.5%.conclusion:LCNEC is a highly aggressive neoplasm with poor prognosis, high tendency of invasion and metastasis. Different stage, age, adjuvant chemotherapy may affect their prognosis.
