ObjectivePeri-implantitis is one of the most common complications in dental implantrestoration, and it can lead to absorption of alveolar bone around the implant.Imbalance between the plaque biofilm and the host immune system causesover expression of inflammatory cytokines and the alveolar bonedestruction.RANKL is a marker of connective tissue and bone destruction whichis binding to RANK directly in the surface of the osteoclast(OC),and stimulatesdifferentiation, maturation of osteoclast precursor cells, and activates the activityof osteoclast.OPG is a competitive receptor of RANKL,through binding toRANKL of OC surface to prevent RANK binding to RANKL. This study was toinvestigate the level of RANK and OPG and ratio of RANKL/OPG in mild,moderate and severe peri-implantitis patients and healthy patientsperi-implantsulcus fluid with non-invasive method, and to statistically analyze the level ofRANKL, OPG and RANKL/OPG ratio in mild, moderate and severeperi-implantitis patients peri-implant sulcus fluid, in healthy implant restorationpatients peri-implant sulcus fluid, combine with clinical index of PPD, mSBI,mPLI, PISF, DSB/LOI and their statistical correlation, peri-implantitis patients,toprovide a data basis for clinical diagnosis and treatment of peri-implantitis.MethodsThis study was divided into four groups: mild peri-implantit is group (N=9),moderate peri-implantit is group (and N=12), severe peri-implantit is group (N= 7), control group: healthy implant restoration(N=24).PISF collecting methods:Before collecting PISF, recorded improved plaque index (mPLI), isolatedmoisture, dried the gingival of peri-implant to prevent saliva contamination,sterilized filter paper was placed gently in the mesial buccal side implant sulcusfor30s. The strip specimens obtained were put into-80°C refrigerator for lateruse, Determination of GCF levels of RANKL and OPG with enzyme-linkedimmunosorbent assay,the experimental results was expressed with x±s,andSPSS13.0statistical analysis software package was adopted to analyzed forvariance test, and correlations of PPD, mSBI, mPLI, PISF, DSB/LOI withRANKL, OPG, and ratio of OPG/RANK were analyzed respectively usingPerson correlation coefficient analysis.ResultsThere were significant differences in average concentration of PPD, mSBI,mPLI, PISF, DSB/LOI, RANKL and in ratio of RANKL/OPG in each group(P<0.01).There was no statistically significant in OPG average concentration ineach group(P=0.43). There were correlations between PPD, mSBI, mPLI, PISF,DSB/LOI and RANKL and ratio of RANKL/OPG respectively using Personcorrelation coefficient analysis. There was no correlation between PPD, mSBI,mPLI, PISF, DSB/LOI and OPG respectively with Person correlation coefficientanalysis.ConclusionsThe results of this study found that RANKL concentration increased andOPG concentration desceased in peri-implantitis patients PISF, indicating thelocal imbalance of OPG and RANKL level in bone reconstruction surroundingimplants, and bone absorption occurred. Positive correlation between RANKLand DSB/LOI was showed, and there was no correlation between OPG andDAB/LOI, this proved that there is compective role when OPG and RANKL arebonding to RANK.
