Laparoscopic Appendectomy Versus Open Appendectomy:a Comparative Study

Objective:To evaluate the advantages and disadvantages of Laparoscopic appendectomy (LA) in the treatment of appendicitis. Methods:A retrospective study was used to analyze clinical data of the137cases who were undergoing LA and256cases who were undergoing OA during August2011and August2013, in our hospital. Compared with the data of operation time, blood loss, length of hospital stay, painkillers used, hospital costs, incidence of complications (incision infection, celiac abscess, intestinal obstruction).Results:In LA group,137cases of laparoscopic surgery,7of which were conversed to laparotomy, and the Conversion rate was5%, among them3cases of appendix root gangrene,2cases of retroperitoneal appendix,2cases of appendix inflammation around the parcel serious edema,6-9days after discharge; There was significantly longer operative time(42.1+/-18.5min in LA and31.5+/-11.5min in OA),less need for analgesia (11.7%in LA and31.3%in OA),more hospital costs(8471+2658yuan in LA and4835+2053yuan in OA, p<0..05)in the laparoscopic appendectomy group. There was no significantly difference in blood loss(4.64+/-2.2ml in LA and4.45+/-2.3ml in OA, p>0.05),postoperative hospital stay(4.64+/-2.2d in LA and4.4+/-2.8d in OA, p>0..05)between two groups, Total incidence of complications was less in the laparoscopic group (2.9%in LA and3.2%in OA); however, there was no statistically significant difference. Conclusion:laparoscopic appendectomy is a safe and clinically beneficial operative procedure in all types of appendicitis. It provides certain advantages over open appendectomy, include a smaller incision, less pain, and smaller skin scar, and particular role in suspected appendicitis diagnosis, but longer operation time and higher cost of hospitalization should be considered. in view of the above characteristics,which type choose should be decided by the doctors and patients together.
