With the deepening of China’s economic system reform, Shanghai is going to reach the goal of "four centers" which included with international economic, financial, trading and shipping center construction, In recent years, regional economic exchanges with other countries are increasingly, which attracting a large number of foreign talents to work and live in Shanghai and surrounding cities.The latest figures from Shanghai statistics Bureau showed the number of expatriates in Shanghai is already ranked first in China, foreign population in Shanghai has reached152,100people by the end of2010, accounting for0.8%of the resident population; the foreign resident’s age range from30to59years nearly55.9%during the total foreign population. It is speeding up the pace of development that Shanghai is as an international metropolis; this number will continue to grow rapidly.Currently, it sets up for expatriates living, the area of work and ancillary service facilities gradually improves in Shanghai. However, due to Shanghai’s living environment, cultural background, values, language, etc. especially the service mode of medical and health care institutions is different with foreign, it makes foreign people to feel uncomfortable the service environment of public hospital in Shanghai uncomfortable. In addition, in order to meet the foreign population medical services the demand of diversified health care. It’s requested that the establishment and development of Shanghai international medical and health service agencies and service mode in line with international standardsAccording to incomplete statistics, it estimated300medical institutions have carried out various forms of joint venture cooperation business in China in2007. There were25Sino-foreign joint venture and cooperative medical institutions by2010; it has increased of32%compared with2005. Sino-foreign joint venture medical institutions will present a rising trend due to the new health care policy, but the joint ventures or cooperative joint venture medical institutions internal operation, is worthy of in-depth analysis.PurposeThis study aims to case study of A Joint Venture Medical Institutions (hereinafter referred to as A Medical Institutions) in Shanghai, analysis resources status, the service mode of A Medical Institution, medical service quality evaluationand and internal operation to find the problems and difficulties during the development. Provide some reference and advices to the executives, investors, and health policy makers. Contents and methodsThis study chose A Medical Institutions between2009and2012yearly basic configuration, medical service quality and financial revenues and expenditures and other data to analysis of the internal operation. The methods were using literature review, retrospective data review, questionnaire investigation, and interviewing method.Results1. The basic configuration of the situationStudies have shown that A Medical Institution personnel26.77%growth in2009to2012. Foreign medical personnel were account for25.21%, the Chinese medical personnel were account for75.89%of the total health technical personnel in2012. Physician and nurse ratio was1:1.69, physician and medical technician ratio was1:0.92, and overall health technical personnel configuration has certain advantages. A Medical Institution of the fixed assets of89.046million RMB, the fixed assets has included medical equipment and office quipment, the amount of medical equipment was73.453million RMB, it was accounting for85.36%of total fixed assets, the amount of office equipment was12.593million RMB, it was accounting for14.64%of total fixed assets in2012. Fixed assets have increased24.32%from2009to2012. Currently, A Medical Institution of medical equipment can meet the needs of medical treatment. 2. Analyses of medical business stausStudies have shown that A Medical Institution outpatient and inpatient load have increased by30.14%~118.15%from2009to2012, the service client were foreigners and domestic high-end people. Foreigners’ have accounts for78.94%of the total service group. Domestic high-end personage have accounted for2.56%~2.56%of total services group, domestic high-end personage is rising trend year by year. Total revenue has increased by31.52%from2009to2012in A Medical Institutions. The revenue of medical has increased by19.43%, medicine has increased by121.96%. Medical expenditure increased by62.48%from2009to2012. The cost of medicine rose by93.18%, repair fee increased by73.10%, the cost of medical materials of health growth of60.20%, personnel expenditure rose by59.89%. As financial report, A Medical Institution capital turnover is good, has the development potential.3. Training and discipline buildingStudies has shown that from2009to2012, A Medical Institutions nurse training of the total number have increased35times; physician training of the total number have increased20times; medical technician training number have increased12times; those logistics personnel training have increased48times; administrative personnel training have increased60times, the discipline construction was serious insufficient. 4. The evaluation of medical service qualityResearch shows that satisfaction survey in2009, the patient was satisfied and very satisfied with the top three, nurse’s service, doctor’s service, and customer service service, the percentage was99.45%,99.17%and98.33%. Patient was the most not satisfied with the service of the medical expenses and auxiliary departments; Satisfaction survey in2012, the patient was satisfied and very satisfied with the top three, customer representative, appointment arrangement, and the doctor’s service, is97.78%,96.39%and95.28%, the most not satisfied with the service of the vaccine and medical expenses.5. Analysis of the interview resultsIt has shown that there were about55.56%interviewees have understood the basic of operation of the joint venture medical institution. There were61.11%interviewees which they suggested govement should plan the development of the joint venture medical institutions; there were77.78%interviewees think that the development of the joint venture medical institutions have a positive impact on health system; there were72.22%interviewees think the government should strengthen the guidance and support joint venture medical institution. ConclusionThrough the research, the following conclusions:1. Medical collectivize management mode is worthy of further promotion.2. To establish talent training and introduction mechanism to meet high-end medical service development.3. It impact the operation of hospital that the effective of internal communication andcoordination in the cross-cultural hospital.4. The efficiency of A medical institutions internal changes, mainly reflected in the composition of service people change and growth in demand for specialist medical services, significantly higher economic income and income structure optimization.5Cooperate with high-end commercial health insurance and the service mode is worthy of further promotion and discusstion.6. The reform of medical system created good conditions for joint venture medical institutions.
