| Objective To investigate the effect of different levels of uric acid(UA) on new onsetdiabetes in hypertension and non hypertension group.Methods A total of101510subjects took part in the health examination during2006to2007from the employees of Kailuan Group.85476subjects who were withFBG<7.0mmol/L, no history of diabetes, complete FBG and UA examination data wereselected as the observation cohort.85476subjects with FBG<7.0mmol/l, without historyof diabetes and usage of hypoglycemic agents were enrolled as the observation cohort.Subjects with incomplete data of FBG and loss of follow up were excluded. There were73279subjects remained in final analysis. The quintiles of uric acid were determined inhypertension and non hypertension group in different gender, and the incidence of diabetesbetween hypertension and non hypertension group in different gender were compared. Therelationship between serum uric acid and NOD was estimated by multiple cox proportionalhazard models.Results1There were73279subjects remained in final analysis. The quintiles of uricacid were determined in hypertension and non hypertension group in different gender,and the incidence of diabetes between hypertension and non hypertension group indifferent gender were compared. Followed up to2010~2011, average follow-up time ofthe obserational cohort was3.95±0.52years,4950cases became diabetes, the incidencewas17.1(/1000person-year). By gender stratification: among female,771casesbecame NOD, the incidence of NOD(/1000person-year) in non hypertension andhypertension group was3.9,4.6,7.4,6.2,7.9and4.3,3.2,5.3,4.7,11.8respectively; amongmale,4179cases became NOD, the incidence of NOD(/1000person-year) in nonhypertension and hypertension group was8.0,8.2,7.8,2,9.0and10.7,9.8,8.8,9.2,12.5respectively.2The result of multiple cox proportional hazard models showed that: infemale, the OR value(95%CI) of the second, third,forth and fifth group was1.10(0.74~1.65),1.63(1.12~2.37),1.50(1.03~2.20),1.48(1.02~2.15)and0.75(0.48~1.16),1.23(0.84~1.81),0.87(0.59~1.29),1.42(1.01~2.00),respectively, in non-hypertension and hypertension group after adjusting for age,TC and FBG, when compared with the first group. In male, compared with the third group, in which the incidence of NOD is the lowest,the OR value(95%CI) of the first,second, forth and fifth group was1.04(0.90~1.21),1.03(0.88~1.19),1.02(0.88~1.18),1.10(0.95~1.28)and1.15(1.00~1.32)ã€1.16(1.01~1.34)ã€1.01(0.87~1.16)ã€1.13(0.99~1.28),adjusting for age, TC and FBG.Conclusion High uric acid increased the risk of developing diabetes in femal, whether innon-hypertension or hypertension groupï¼›In hypertension group while low uric acid(<233μmol/L)may increase the risk of developing diabetes in male. |
