This study was divided into two parts. In the first part, investigated the cutoffvalue of blood perfusion for normal lung parenchyma at breath-hold status and theinfluence of gravity factor on it. In the second part, investigated the influence ofperipheral lung tumors on the blood perfusion of lung tissue around the tumor andthat clinical significance.PartⅠ The Characteristics of Blood Perfusion for Normal Lung ParenchymaObject The aim of this study was that use of the contrast agent first-pass CTperfusion (CT perfusion imaging, CTP) scanning technology to evaluat the cutoffvalue of blood perfusion for normal lung parenchyma at breath-hold status and theinfluence of gravity factor on it. Methods Prospective study in77patients whoselung with solitary pulmonary nodules in our hospital, that left lung in38cases and39cases of right lung. According to the positioning of CT perfusion scans obtainedbefore, The lung tissue was dividede into three parts that the upper, middle and lowerfield, then each field was divided into ventral, axillary and back side. The side of lungwhich without pulmonary nodules as normal. As the pulmonary nodules was center,the selected aspects were scande by multislice spiral CT perfusion. Normal lungperfusion parameters were analyzed by Perfusion3-body tumor software of GE ADW4.3workstation. Sub pleural1cm for the interested area by scanning the lower leveloffice of the pleura. The blood flow (BF), blood volume (BV), mean transit time(MTT), permeability surface (PS) and other perfusion parameters were measured inthe ventral, axillary and back side of the upper, middle and lower lung fields In the77cases of health lung tissue. To compare the parameters variations for same side of theventral, axillary and back side in same lung tissue and different, evaluate theinfluence of gravity factor. Groups were compared with ANOVA. Results Normal pulmonary blood flow distribution in the supine position of the heath lung tissue, allventral BV and BF perfusion values were lower than the dorsal of the upper, middleand lower lung fields.The different was statistical significance(P<0.05). Comparedwith the same side of the heath lung tissue, the difference was not statisticallysignifican(tP>0.05). Conclusions Multislice CT perfusion scanning can be used toevaluate the distribution of pulmonary blood flow. Due to the effects of gravity factors,corre-sponding changes of the perfusion in normal person will occur. Methods The study need47patients who diagnosed peripherallung cancer but without accepected any treatments, including25cases of lung canceraround the left and right peripheral lung cancer in22cases. According to thepositioning of CT perfusion scans obtained before, the lung tissue was dividede intothree parts that the upper, middle and lower field. As the pulmonary nodules wascenter, the selected aspects were scaned by multislice spiral CT perfusion. Normallung perfusion parameters were analyzed by Perfusion3-body tumor software of GEADW4.3workstation.47cases patients of peripheral lung cancer. Blood flow (BF),blood volume (BV), mean transit time (MTT), permeability surface (PS) and otherperfusion parameters were measured for the tissue of peritumoral mediastinal side anddistal lung, and with the corresponding region of the contralateral heath lung tissue.Evaluated the differents of perfusion parameters between the two sides. Comparisonbetween groups using independent sample t test. Results BV and PS perfusionvalues of lung parenchyma around peripheral lung tumor were lower than the healthylung in both regions, the differences were statistically significant (P<0.05).Conclusions Multislice CT perfusion scanning can be used to evaluate blood flow.There were significantly low blood perfusion in peripheral lung cancer.
