Background:Heart disease in pregnancy is the first leading cause of maternaldeath among the non-obstetric causes in China. For the pregnant women complicatedwith congenital heart disease(CHD), the maternal mortality and the risk of fetal andneonatal complications are obciously higher than the ordinary healthy women.However, it is still unknown whether umbilical cord blood gas values are differentbetween pregnant women complicated with CHD and healthy women undergoingelective cesarean section during combined epidural-spinal anesthesia(CSE). Thepresent study focused on the umbilical cord blood gas analysis, expecting to finddifference and guide for the clinical treatment.Methods:Patients with singleton full-term pregnancy undergoing elective cesareandelivery under combined spinal–epidural (CSE) anesthesia were enrolled. Cases weredivided into two groups: healthy pregnant women (Group H) and pregnant complicatedwith CHD (Group CHD). We collected the umbilical cord arterial and venous bloodsamples immediately after delivery and also the maternal arterial blood, all sampleswere meatured at once. We used grouped t test to compare the gas values of twogroups.Results:44cases in Group H and33cases in Group CHD were enrolled at last.The values of umbilical arterial blood pH, BE and HCO3-(pH7.29±0.05, BE-1.4±1.9 mmol/L, HCO3-21.5±1.4mmol/L) of the Group CHD were apparent lower than thoseof the Group H (pH7.33±0.03, BE-0.1±2.1mmol/L,HCO3-22.5±1.4mmol/L)。Comclusions:The values of umbilical arterial blood pH, BE, and HCO3-ofpregnant women complicated with CHD showed lownesses compared with the valuesof the healthy pregnant women, when undergoing elective cesarean delivery undercombined spinal–epidural anesthesia.
