Objective: The objective of this study was to identify the possibility of usingbiomarkers as conjunctive standards to classify histopathology lesions thatmorphologically would earlier have been diagnosed as CIN2to LSIL or HSIL,whichmay lead to improvement in managing young women with cervical high-gradesquamous intraepithelial lesions on biopsy.Methods:90cases of cervical biopsies and conization specimens were obtainedfrom Shanghai First People’s Hospital Department of Pathology from the year2012to2013.Of the90cases,30cases of them were from CIN1,which were diagnosed onH&E-stained by two pathologists independently,and also did CIN2and CIN3respectively. We have chosen three differentiation-related biomarkers to addresswhether p16, cyclinD1and survivin immunohistochemical staining could be asadjunctive Standards to help with classifing CIN2to LSIL or HSILResults:(1)The staining of p16in CIN1were2of30(6.6%) negative,9of30(30.0%)weak positive,19/30(63.3%)moderately positive and none ofthem were strongly positive. The staining of p16in CIN2were1of30(3.3%) negative,1of30(3.3%)weak positive,17/30(57.7%)moderatepositive and11of30(36.6%) strongly positive. The staining of p16in CIN3were5of30(16.7%) negative,1of30(3.3%)weak positive,9/30(30.0%)moderate positive and15of30(50.0%) strongly positive.(2)The staining of survivin in CIN1and CIN3were both16of30(53.3%)negative,18of30(60.0%)weak positive, and none moderately or stronglypositive stained. The staining of survivin in CIN2were18of30(60.0%)negative,12of30(40.0%)weak positive, and no moderately or stronglypositive stained.(3)The staining of cyclin D1in CIN1and CIN3were both only1of30(3.3%) weak positive, all of the rest are negative.Conclusion:(1) Using the p16INK4a immunohistochemical staining of strongly positiveas the cut point may help with classifying CIN2to LSIL or HSIL.(2) No difference of expression were observed both in survivin and cyclinD1,which means p16is better than the other two in classifying CIN2toLSIL or HSIL when has been.
