Comparative Study Of Bile Duct Single Stents And Biliary And Pancreatic Duct Stent For The Curative Effect Of Advanced Pancreatic Head Carcinoma

Objective To compare the curative effect of advanced pancreatic head carcinoma by bileduct single stents and biliary and pancreatic duct stent.Materials and Methods We conducted a retrospective review of90patients withadvanced pancreatic cancer between June2010and June2013of Ningxia Medical University.We divided the patients into two groups, single stent group and biliary and pancreatic ductstent group.42cases received bile duct single stents and48cases received biliary andpancreatic duct stent. We compared the changes in liver function，abdominal pain, quality oflife scores after treatment between two groups. We collected incidence of postoperativecomplications, length of stay, hospital costs and other data collation. According to the data,we compare the curative effect of two kinds of treatments.Result In the single stent group, the biliary stents were placed successfully in39patients(93%). In the double stent group, the biliary and pancreatic duct stents were placedsuccessfully in45patients(94%). The total bilirubin and other parameters of liver functionwere decreased significantly compared with those of pre-ERCP in both of the two groups. Butthere was no significant difference between the two groups (P>0.05). The overall pain reliefrate and complete pain relief rate in double stent group were significantly higher than those insingle stent group(75.6％vs．43.6％；35.6％vs．17.9％, P<0.05). At14d after ERCP,Kamofsky QOL score were improved significantly in double stent group,[(84.6±12.2)vs．(71.3±14.6)]. It was significantly better than single stent group at14d afterERCP(P<0.05)． Conclusion In the ways of improving the jaundice and liver function of patients withpancreatic cancer, it is the same in single stent and double stent. But, in the ways ofalleviating obstructive symptoms, alleviating pain and improving quality of life scores, it wasbetter than ERCP biliary stent.
