ABSTRACTObjectives:Oxidative stress, inflammation and sleep disorders exist in maintenance hemodialysis(MHD) patients. Melatonin secreted by pineal gland regulates circadian rhythm and possesses potent anti-oxidative and anti-inflammatory activities. In the present study, the levels of serum melatonin, oxidative stress and inflammation in MHD patients were investigated and further the correlation analysis between serum melatonin and oxidative stress, inflammation and sleep quality was performed.Methods:42 MHD patients were used to collect the fasting blood serum before dialysis. 20 healthy volunteers were used as control. The levels of MT and IL-6 were detected by ELISA. The levels of MDA, NO, SOD, and GSH-Px were determined by colorimetric assays. The levels of renal function and CRP were examined by automatic biochemical analyzer. The sleep quality was evaluated using PSQI.Results:1 Compared with the control, the levels of MDA(5.75±1.35 VS 2.80±0.51nmol/L) and NO(27.62±17.32 VS 10.82±4.51umol/L) were significantly increased, the levels of MT(20.30±2.87 VS 24.85±2.51pg/m L)、SOD(74.40±11.07 VS 85.90±10.44U/m L) and GSH-Px(65.56±14.67 VS 79.26±14.61 U/m L) were all significantly decreased, the levels of IL-6(24.76±4.43 VS 9.59±1.01pg/m L) and CRP(8.51±6.56 VS 2.41±0.09mg/L) were significantly increased in MHD patients. PSQI was 17.5±1.29 in MHD patients. These results indicated that the level of MT was decreased and the level of oxidative stress and inflammation were increased in MHD patients. Moreover, MHD patients had sleep disorders.2 Serum MT was used as dependent variable and the multi-element linear regression analysis between serum MT and PSQI score and the levels of MDA, NO, SOD and GSH-Px were carried out. The multiple regression equation: MT=20.587-1.209MDA-0.012NO+0.076SOD+0.02GSH-Px. The results indicated that MT might be negatively correlated with MDA and NO, and might be positively with SOD and GSH-Px.3 Serum MT was used as dependent variable and the multi-element linear regression analysis between serum MT and IL-6 and CRP were carried out. The multiple regression equation: MT=30.126-0.331(IL-6)-0.192 CRP. The results indicated that MT might be negatively correlated with IL-6 and CRP.4 Serum MT was used as dependent variable and the linear regression analysis between serum MT and PSQI. The regression equation: MT=29.207-0.65 PSQI. The results indicated that sleep disorders in MHD patients might be due to the decreased serum MT.Conclusions:1 The level of MT was significantly decreased and the level of oxidative stress and inflammation were significantly increased in MHD patients. Moreover, MHD patients had sleep disorders.2 MT was negatively correlated with oxidative stress, inflammation and PSQI. These results suggest that the decreased serum MT might be due to oxidative stress and inflammation, and sleep disorders in MHD patients might be due to the decreased serum MT.
