Objective:Cholangiocarcinoma is one of malignant tumor,which is seriously harmful to human health, the incidence and mortality of patients are gradually rising.Because of cholangiocarcinoma onset secretly,there is not obvious clinical manifestations in early cholangiocarcinoma, patients can be payed attention when occured biliary obstruction clinical symptoms, and has missed the best opportunity to surgical treatment, the clinical effect and prognosis is poor. Study cholangiocarcinoma pathogenesis, make a diagnosis in the early, achieve good clinical treatment effect, more and more be concerned.Osteopontin(OPN) is a kind of calcium binding phosphorylated glycoprotein, and has been found in the human body many different organizations. OPN has two kinds of receptor: integrin receptor and receptor CD44, OPN combined with receptors, through specific signaling pathways regulate cell adhesion and migration,participate in tumor cell proliferation, infiltration and other pathological process. CD44 is a transmembrane glycoprotein on the surface of the cells in the human body, and play an important role in the adhesion process between cells and cells, cells and matrix. CD44v6 is one of the main allosteric body of cell surface adhesion molecules CD44,can affect the cell adhesion function, strengthen cell migration、movement ability, realize the invasion and metastasis of tumor cells.In this paper, we detectived OPN and CD44v6 expression in cholangiocarcinoma tissues,bile duct tissue adjacent to carcinoma,normal bile duct tissues,understood the relationship between OPN、CD44v6 and cholangiocarcinoma,and provided the molecular level evidence for the study of occurrence and development of cholangiocarcinoma.Methods:Collected 46 cases resection specimens of pathology confirmed cholangiocarcinoma of Baoding First Hospital from January 2008 to October 2014,including 24 males and 22 females, aged 35-72 years old,the average age of 62.5 years. Pathological types including:high differentiatied adenocarcinoma 17 cases,moderately differentiated adenocarcinoma 18 cases,and poorly differentiated adenocarcinoma 11 cases.27 cases with lymph node metastases, 19 cases without lymph node metastasis.According to the 2010 edition Union for International Cancer Control(UICC) staging method, including:Ⅰstage 8 cases,Ⅱstage 13 cases,Ⅲstage 19 cases,Ⅳstage 6 cases. Specimens were routinely drawn and fixed by 0.5% formaldehyde, conventional paraffin-embedded,4 microns thick serial section.And collected 46 cases of bile duct tissue adjacent to carcinoma and 20 cases of normal bile duct tissues polyps for comparison.All specimens were detected OPN and CD44v6 expression by immunohistochemical Elivision method. The experimental data results were analyzed by SPSS17.0 statistical software. The research data was counted using χ2 test,when a certain theoretical number 1<T≤5,χ2 data would be continuously corrected,If P<0.05, then it would be considered as statistically significant. The correlation of OPN and CD44v6 use the Spearman rank correlation analysis.OPN and CD44v6 expression in cells position observation:OPN positive expression is yellow or brown granules in the Cell cytoplasm, flake or nests distribution, yellow granules can be seeing occasionally in some of the lumens epithelial cells; CD44v6 positive expression is in the cell membrane, Dyeing is tan,light yellow can be seen in cell cytoplasm occasionally.Please two professional pathologist observe each slice dyeing conditions independently,and judge results synergistically.Results: OPN is expressed in 31 cases cholangiocarcinoma out of the 46 cases cholangiocarcinoma tissues,there are 5 cases positive expression out of the 46 cases bile duct tissue adjacent to carcinoma and 2 cases positive expression out of 20 cases normal bile duct tissues.The positive expression rate are 67.4%,10.9% and 10.0% respectively.In the cholangiocarcinoma tissue and normal bile duct tissues OPN expression difference is statistically significant(P<0.001),In the cholangiocarcinoma tissue and bile duct tissue adjacent to carcinoma OPN expression difference is statistically significant(P<0.001).CD44v6 is expressed in 34 cases cholangiocarcinoma out of the 46 cases cholangiocarcinoma tissues,there are 8 cases positive expression out of the 46 cases bile duct tissue adjacent to carcinoma and 3 cases positive expression out of 20 cases normal bile duct tissues. positive expression rate are 73.9%, 17.4% and 15.0% respectively. In the cholangiocarcinoma tissue and normal bile duct tissues CD44v6 expression difference is statistically significant(P<0.001),In cholangiocarcinoma tissue and bile duct tissue adjacent to carcinoma CD44v6 expression difference is statistically significant(P<0.001).OPN and CD44v6 expression in cholangiocarcinoma tissue are not related with patient’s age, sex, tumor differentiation degree(P>0.05), are related with tumor stage and lymph node metastasis. OPN positive expression rate in stage Ⅲ、Ⅳ(84.0%)of cholangiocarcinoma tissues is significantly higher than that in stageⅠ、Ⅱ(47.6%),OPN positive expression rate in the cholangiocarcinoma tissues with lymph node metastasis( 85.2%) is significantly higher than that without lymph node metastasis(42.1%),all P<0.05. CD44v6 positive expression rate in stage Ⅲ、Ⅳ( 88.0%) of cholangiocarcinoma tissues is significantly higher than that in stageⅠ、Ⅱ(57.1%),CD44v6 positive expression rate in the cholangiocarcinoma tissues with lymph node metastasis(88.9%)is significantly higher than that without lymph node metastasis(52.6%),all P<0.05.26 cases out of the 46 cases of cholangiocarcinoma OPN and CD44v6 expression are simultaneous positive, 5 cases are OPN positive expression while CD44v6 negative expression. 8 cases are OPN negative expression while CD44v6 positive expression. 7 cases OPN and CD44v6 expression are simultaneous negative.The Spearman rank correlation analysis show that there is positive correlation between the expressions of OPN and CD44v6 in cholangiocarcinoma(r=0.326, P<0.05).Conclusions:1 OPN and CD44v6 expression are Significantly higher in cholangiocarcinoma tissue, and are important biological indicators of cholangiocarcinoma.2 OPN and CD44v6 expression in cholangiocarcinoma tissue are not related with with patient’s age, gender, tumor differentiation degree, but related with tumor clinical stage and lymph node metastasis. OPN and CD44v6 positive expression rates in stage Ⅲ、Ⅳ of cholangiocarcinoma tissues are significantly higher than that in stageⅠ、Ⅱ,OPN and CD44v6 positive expression rates in the cholangiocarcinoma tissues with lymph node metastasis are significantly higher than that without lymph node metastasis.3 OPN and CD44v6 expressions are positive correlation in cholangiocarcinoma tissues,The joint detection is important for early diagnosis and prognosis judgement of cholangiocarcinoma.
