Objective:Through studying the clinical data and postoperative pathological data after radical resection of the bladder cancer,the correlation between pathological stage,grading of bladder cancer and lymph node metastasis is analyzed.And choose the suitable treatment according to its correlation.Evaluation the accuracy of CT and MRI in predicting lymph node metastasis of bladder cancer.Methods:This study collects all of the bladder cancer patients who underwent radical cystectomy and lymph node dissection from January 1,2017,to December 31,2018,at the Urology Surgery of second affiliated Hospital of Kunming Medical University.And the postoperative clinicopathological data has confirmed the papillary urothelial carcinoma.These data include the sex,age,total number of resected lymph nodes,number of positive lymph nodes,tumor pathological grade,staging,preoperative CT and MRI report.Through the statistical analysis,get the correlation between lymph node metastasis and pathological stage and grade of bladder cancer.And then,the most suitable choice for the patient's treatment method in the future will be given according to its correlation.The evaluation of the accuracy in predicting lymph node metastasis of bladder cancer by preoperative CT and MRI,together with the guidance of the scope of intraoperative lymph node dissection,will be conducted.Results:The average age for the 159 bladder cancer patients were 64.5±7.3 years old,There were 139 males and 20 females,26 cases were pathological positive of lymph nodes after operation.Through analyzing the post-operation tumor histology and resected lymph nodes,it is considered that the lymph node metastasis of bladder cancer is related to its pathological grade and stage.Preoperative CT and MRI can be used for staging and grading of bladder cancer.And lymph node metastasis of bladder tumor can be predicted before the operation,but the specificity and sensitivity of the two methods are different.Conclusion:Through the retrospective study,it is confirmed that lymph node metastasis is rare in low-grade uroepithelial carcinoma of the bladder.Lymph node metastasis may occur in high-grade uroepithelial carcinoma of the bladder when the pathological stage is T1.The higher the pathological stage is,the higher the probability of lymph node metastasis will be.There is a positive correlation between them.When evaluating the lymph node metastasis of bladder cancer before the operation,MRI is obviously superior to CT,and MRI can make a more accurate staging of bladder tumor before the operation.Therefore,it is suggested that a routine MRI examination should be performed before the operation for predicting the possibility of lymph node metastasis and guiding the lymph node dissection.
