Application Of Three-Dimension Optical Coherence Tomography On Measuring Macular Ganglion Cell Complex Thickness In Diagnosis Of Early Primary Open Angle Glaucoma

Obtective To compare the difference of macular ganglion cell complex(mGCC) thickness and peripapillary retinal never fiber layer(pRNFL) thickness between normal and early primary open angle glaucoma (POAG) measured by 3D optical coherence tomography (3D OCT),and analyze the diagnostic value of these on early POAG.Methods Thirty early POAG cases(30 eyes) and 33 normal cases (33 eyes) were respectively enrolled and set as experimental group and control group. The thickness of pRNFL in superior,inferior,nasal,temporal,average and the thickness of the macular nerve fiber layer (mRNFL), ganglion cell laye r combing the inner plexiform layer(GCIP), and ganglion cell complex (GC C) in average,superior,inferior were recorded by 3D OCT(Topcon 3D OCT-2 000 Ver 8.0), and the parameters were analyzed statistically.The ability of ea ch parameter to discriminate early POAG was assessed by analyzing the are a under the receiver operating characteristics curve(AUROC).Results The thicknesses of mGCC and pRNFL at all parts in experime ntal group were less than that in control group,all parameters were significa nt differences except for the nasal (t=0.633、P=0.529) and temporal(t=1.020、 P=0.312) pRNFL parameters.The AUROC of mRNFL,GCIP,GCC in superior,i nferior,average and pRNFL in superior, temporal,inferior,nasal,average of earl y POAG were 0.798,0.701,0.808,0.783,0.777,0.794,0.823,0.756,0.824,0.809,0.5 36,0.828,0.533,0.811,respectively.The parameters showed significantly positive diagnostic value for early POAG (P<0.05),except for the nasal and tempora 1 pRNFL(P=0.693,0.666 respectively).The most effective parameters in the ab ility to diagnose early POAG was inferior pRNFL(0.828),which had the sam e diagnosing ability as average GCC(0.824),superior GCC(0.823),average pR NFL(0.811),superior pRNFL(0.809),average mRNFL(0.808) and average GCIP (0.794),there was no statistical difference (all P>0.05).Conclusions The changes of mGCC and pRNFL thickness with early POAG were effectively detected by 3D OCT.The capacity of mGCC parameters to diagnose early POAG were similar to that of the pRNFL.
