Background With the improvement of living standards and science and technological progress,glaucoma has become the major blinding eye disease. The early detection of glaucoma is particularly important,Retinal nerve fiber layer thickness measurements have a certain value for the early detection of glaucoma.In this study, using OCT measurement of macular ganglion cell complex, Investigate the correlation between macular ganglion cell complex thickness and peripapillary retinal never fiber layer thickness and significance in Glaucoma diagnosis.Objective To compare the glaucoma differentiated ability of macular ganglion cell complex (mGCC) and peripapillary retinal never fiber layer (pRNFL)in normal,early glaucoma and advanced glaucoma using Fourier-domain optical coherence tomography (FD-OCT).Investigate the correlation between macular ganglion cell complex thickness and peripapillary retinal never fiber layer thickness in various parts of the normal group,early glaucoma group and advanced glaucoma group.To evaluate and compare the early glaucoma diagnostic capabilities.Method41normal subjects (56eyes),36early glaucoma subjects (42eyes) and35advanced glaucoma subjects (39eyes) were enrolled in the study.The thickness of mGCC and pRNFL were measured using FD-OCT:the ONH protocol (NHM4) measure the the of Peripapillary3.4mm diameter thickness,the GCC protocol measure the of thickness macula ganglion cells complex.Scanning and recording Various parts of the pRNFL the thickness and mGCC thickness in normal group,early glaucoma group and advanced glaucoma group. Contrast between various parts pRNFL thickness and macular GCC thickness in each group using SPSS17.0statistical software. To pearson related correlation analysis correlation,To evaluate the diagnosis of glaucoma using Area under Receive Operated Curve (AROC).Result The normal group,early glaucoma group and advanced glaucoma group patients which were significantly different pRNFL and mGCC thickness among the three groups (F=212.07,85.19,138.96,38.10,123.94,134.88,124.30,112.34,P<0.01).The parts of the pairwise comparisons showed no significant difference between the groups except nasal(p=0.120)in normal group and early glaucoma group. The rest of the superior, the inferior, temporal of pRNFL parameter and superior, inferior, the average of mGCC parameter were statistically significant (P<0.01).In all groups mGCC thickness and pRNFL of thickness have a good correlation, the correlation coefficients were are0.433to0.904. the correlation coefficients were0.433,0.693,0.615,0.833,0.725,0.818,0.840,0.658and0.904among average,superior and inferior of normal group,early glaucoma group and advanced glaucoma group. Area under Receive Operated Curve (AROC) of all pRNFL parameter in early glaucoma except nasal (P=0.102) and mGCC parameters had significantly diagnostic capability (pRNFL:0.890±0.035,mGCC:0.856±0.037,P<0.01).As to the whole glaucoma,AROC of pRNFL and mGCC parameters had significantly diagnostic capability (pRNFL:0.941±0.020,mGCC:0.920±0.022,P<0.01). Through AROC pairwise comparison,the diagnostic power of all pRNFL and mGCC parameters had no difference(P>0.05).Conclusion The ability to diagnose glaucoma with macular GCC thickness is comparable to that with peripapillary RNFL thickness.Macular GCC thickness measurements may be a complementary measurement to RNFL thickness assessment in clinica evaluation of glaucoma.
