The Analysis Of Urinary Calculi Composition Of Servicemen In The Northwest Region And The Research Of TCM Preventing Urinary Calculi On Rats

ObjectiveTo detect the urinary calculi morbidity, crowd and chemical composition of srvicemen in Northwest Region inpatients, contrast the urinary calculi with other regions,and explore the possible cause so that propose effective measures to prevent urinary calculi.Besides, the mixture of the Fructus Lycii and Alismatis Rhizoma was applied on rats to observe the effect on renal calcium oxalate stone formation.Methods1. Selected medical records of front page information database of servicemen inpatients of the General Hospital of Lanzhou Military Region,The First Hospital of PLA and The Fourth Hospital of PLA from 2008 to 2012,according to ICD-9 to systematically classifed,statistic analysis and top 5 single disease descriptions.Moreover, a total of 398 servicemen samples of urinary calculi from inpatient department were analyzed by LIIR-20 calculus infrared spectrometry autoanalyser.2. Adult male SD rats were randomly divided into group A, B, C, D, E, eight of each group. Group A was the control group and given saline every day. Group B,C,D,E were given ethylene glycol and ammonium chloride to induce urolithiasis.Meanwhile,Group C,D,E received Fructus lycii infusion,the total triterpenoids extract and mixture of them, respectively.After treatment of 28 day, blood and urine biochemical parameters and kidney index were evaluated,besides,the levels of malondialdehyde(MDA) and activity of total-superoxide dismutase(T-SOD) in renal tissues were detected. Kidney sections were examined under a light microscope to study the calcium oxalate crystal deposition and pathological changes.Results1. The incidence of urinary calculi is in the first place.There are 2541 servicemen patients hospitalization for different urologic diseases within 5 years,which ccounting for 12.90% of the total urobgic in-patient number at the same period,the in-patient number of servicemen and the ratio accounting for the total urologic in-patient number of 2008,2009,2010,2011 and 2012 are respectively 653 (18.44%),501 (13.39%),516 (13.43%),462 (11.28%),409 (9.71%).The top 5 single diseases are calculus, varicole,uronephrosis, renal colic,BPH(benign prostate hyperplasia).9 components in urinary stones were detected.113 cases(28.39%)were pure type,while 285 cases(71.61%)were the mixed type.24.87% of the calculi were calcium-containing stones,3.52% of the stones were non-calcium-containing,the rest were mixed.The percentage of various stone compositions were as follows:calcium oxalate monohydrate(COM)+calcium oxalate dihydrate(COD)+carbonate apatite was 35.18%(140cases),calcium oxalate monohydrate(COM) was 24.87%(99cases), calcium oxalate monohydrate(COM)+ carbonate apatite was 11.56%(46cases). The incidence of mixed type is 71.61%,higher than North China(39%) and South China(48.73%),but as for pure type, the incidence of North China is 61.1%,higher than South China(51.27%) and Northwestern Region(28.39%).The incidence,age of onset and composition of Gansu and Qinghai Province showed no significant difference.2.Rats were given ethylene glycol and ammonium chloride and successfully to model calcium oxalate stones.The Fructus lycii,Alismatis Rhizoma and mixture of them can significantly decrease the levels of serum urea and creatinine,urine oxalate and Ca2+,relative kidney weight,renal Ca2+,MDA in renal tissues,and increase activity of T-SOD.The effect of mixture of the Fructus lycii and Alismatis Rhizoma group is better than Fructus lycii group and Alismatis Rhizoma group(P<0.05), there was no statistical difference between Fructus lycii group and Alismatis Rhizoma group(P>0.05).They also diminished the number and size of calcium oxalate crystal deposites and protected the kidney from serious damage.Conclusion1. It sized up the situation about urologic disease and change trend of servicemen inpatients at this area,which provided solid basis for servicemen-service and pointed direction for future construction for urologic department. Besides results of composition analysis of urinary stones could instruct clinical treatment,and guide exercises,diet and living habit of servicemen for prevention of urinary calculi.2. Fructus lycii and Alismatis Rhizoma can both inhibit urolithiasis induced by ethylene glycol and ammonium chloride,and the mixture of them is more effective.
