Objective:To evaluate for the rational use of Traditional Chinese medicine(TCM) injections. study on the cause of adverse reactions of TCM injections. To provide advice and evidence for the rational use and safety of clinical drugs. Methods: After retrieving the database, we were screened and retrospectively analyzed 2648 TCM injections adverse reaction cases;Different specially designed questionnaires were distributed to 90 physicians,90 nurses and 150 patients to test their understanding of TCM injections;Detection was made to test particles in varieties of TCM injections under different configuration conditions, and statistical analysis was made; Detailed information of thirty-five directions of TCM injections were analyzed to calculate mark rate; Medication amount and DDDs of TCM injections at two medical institutions between 2012 and 2014 were also summarized, sorted and analyzed. Results: In 2648 adverse reaction cases, there are 1289 male patients(48.68%)and 1359 female patients(51.32%). The highest incidence of adverse reaction was involved in the skin and its accessories in 1005 cases(35.12%). In 54 kinds of TCM injections, Shuang Huang Lian injection has the highest adverse reaction rate. Out of the 330 questionnaires,319 were filled and returned by 82 physicians, 87 nurses and 150 patients respectively.Particles of TCM injection prepared in a clean environment configuration were significantly less than those in ordinary environment(P <0.05); Instructions of31 TCM injections were analyzed, accessories(51.6%), infusion rate(39.3%),pediatric(35.5%), geriatric medicine(32.3%), drug interactions(9.7%),clinical trials(standard deviation of 19.4%), toxicology(25.8%),pharmacokinetics(3.2%), etc. Cardiovascular category TCM injections in clinical were mostly used in two medical institutions. Conclusion:Clinical adverse reaction of TCM injection has no gender difference and can affect multiple tissues and organs. Shuang Huang Lian oral liquid has the highest alert signal. Therefore, we need to strengthen comprehensive monitoring to find and report adverse reaction. Different kinds of people have different cognition on the pros and cons of TCM injections, and patients’ awareness of TCM injections safety is the worst; Configuration environment has impact on the number of TCM injections of particles. TCM injections in the ward whose solvent is inappropriate are often placed too long time. We propose that some hospital set up the intravenous drugs allocation center, checking the prescription by pharmacists who have systematic pharmaceutical knowledge and working experience, and deploying in strict accordance with standard operating procedures by specially trained pharmacists or nurses. Enterprises need to improve production process and strengthen standardization of instruction. Additionally, the components or accessories containing which may cause adverse reaction shall be specified. National food and drug administration should step up the clinical monitoring and security research on TCM injections. At the same time, it should play management and intervention roles on medical institutions to improve the reasonable application of TCM injections.
