Expression Of Toll-Like Receptor 7,T Lymphocyte Subsets In Skin Lesions From Patients With Condyloma Acuminatum

Backgroud and Objective:Condyloma acuminata(CA) is caused by human papilloma virus infection,which mainly shows verrucous hyperplasia in genital-anal area skin,or performance in oral cavity,airway and other parts.At present,there’s short of specific drugs or methods of resisting to HPV infection. Because this disease is easy to relapse and infectious,it seriously affects the patients’ physical and mental health, and also brings great pressure to social control.There were T lymphocyte subgroups and cytokine imbalance within the CA patients peripheral blood and skin lesions.Due to a series of cellular immune response inhibition effect caused by local immune cells dysfunction,the body does not produce effective immune response against HPV,leading to HPV existing for a long time and CA occuring repeatedly.The body immune response to HPV infection includs inherent immune and adaptive T cells immune, particularly dependenting on the scavenging role of adaptive T cells immune response against HPV infected cells.As an important pattern recognition receptors in innate immunity,Toll-like receptor (TLR) is connected to the inherent immunity and adaptive T cell immune, and that TLR7 agonist imiquimod which treats CA effectively displays TLR7 plays an important role in induction of HPV immunization.On the basis of cell function, the T cell can be divided into helper T lymphocyte(Th),cytotoxic T cell(Tc) and regulatory T cell(Treg or Tr).Tc can kill the infected cells, and Th is to strengthen the Tc and the inherent immune cell function.Treg can regulate the body’s immune response by inhibiting activity effect of Th and Tc,so it’s closely related with persistent infection of virus.While the expression of Foxp3+ is closely related to the differentiation and function of Treg, it is a sign of Treg’s inhibiting function.In other words,we can use Foxp3+ cell to show Treg.Nowadays, reports don’t agree with each other about TLR7 expression in the skin lesions area in patients with CA.Despite reports on T lymphocyte subsets of CA lesion, there is rare on local Treg CA lesion,as well as the ratio of CD4+ and CD8+T cells, and there is more rare Treg/Th or Tr/Tc.By observing the distribution and expression change of TLR7 and the distribution,number and ratio change of Foxp3+Treg,CD4+T,CD8+T cells within CA lesion, this study analyzes the relationship of incidence and outcomes between these T cells and CA.The results of this study maybe provide an experimental data reference for prevention and treatment of CA and other related diseases,and also accumulate some experimental data for the next study.Methods:TO select 30 patients with incipient CA as the experimental g roup,record the number、size and site of wart body,take the lesions after routine disinfection and 2% lidocaine local anesthesia;TO select 12 cases who is healthy as normal control group, take the prepuce inner plate ti ssue after routine disinfection and 2% lidocaine local anesthesia.After bei ng soaked in 10% formaldehyde, all tissue specimens were made into p araffin block.Watch the distribution and expression number of TLR7,CD 4+T,CD8+T lymphocytes and Foxp3+Treg by using immunohistochemical method-Using statistical analysis software support SPSS 13.5 process the experimental data, Because TLR7 can express in a variety of cells, its e xpression score is according to the staining,using rank and inspeetion,and the average rank, said the inspection level is set to the alpha=0.05(bilat eral), namely when P＜0.05 think the difference is statistically significant. The expression of T cell subgroup, its data is count data,the data said with mean ± standard deviation (x±s).Immune status differences between the experimental group and normal control group use independent samp les t test, setting the inspection level to α=0.05 (bilateral), namely when P<0.05 think the difference is statistically significant.Results:1.The expremental group TLR7 mean rank is 26.2,the normal group is 9.75,and the Z is 3.946,significantly enhanced patient group than normal expression.The difference was statistically significant (P<0.05),so the expression of TLR7 is more in the expremental group than in the normal group.2.After HE staining,we detect a large number of lymphocyte infiltration in the dermis of patients with CA,while rare in normal group;Immunohisto chemical staining results showed that CD4+T cell count 27.33±3.66 and CD8+T count 36.48±3.98 in the CA group are more than CD4+T count 14.38±7.07 and CD8+T 9.78±4.73 of normal healthy controls increased significantly,with significant difference (P<0.01);Compared with control group, the ratio of CD4+T/CD8+T decreased,the difference was statistically significant (P＜0.05). 3. Foxp3+Treg number 7.83 ±0.49 of the CA group is significantly higher than in the normal group 1.60+0.94, the difference was statistically significant (P<0.01);Foxp3+Treg/CD4+T 0.29±0.04 and Foxp3+Treg/CD8+T 0.22±0.03 in the CA group are significantly higher than Foxp3+Treg/CD4+T 0.10±0.07 and Foxp3+Treg/CD8+T 0.14±0.09 of the comparison (P<0.05).Conclusion:1.The expression of TLR7 is increased significantly in skin lesions of patients with CA, mainly at the base bottom and stratum spinosum cutin cell formation. It shows TLR7 plays an important role in the resistance of HPV infection.2. It prompts the body launched T cell immune response to HPV,which CD4+T and CD8+T cells increased in lesions local of patients with CA.But the ratio of CD4+T/CD8+T lymphocyte decreased, which shows T cell subgroup exists imbalance;3.The number of Foxp3+Treg and the ratio of Foxp3+ Treg/CD4+T,Foxp3+ Tr-eg/CD8+T increased.lt prompts Foxp3+Treg involved in the regulation of the body’s immune response to HPV infection.That may be one of the important reasons of low effect of the body’s immune response to HPV infection.
