bjective: To investigate the role of lymphatic drainage in the endotoxin-induced lung injury at early stage of abdominal infection and the effects of regulation of the peritoneal lymphatic stomata on endotoxin induced lung injury.Methods : Part 0ne:Seventy-two healthy male Wister rats of clean grade were divided into 3 groups: control group(group SO, n=24), underwent laparotomy followed by flipping the cecum and injection with physiological saline; the experimental group(group CLP, n=24), underwent cecal ligation and perforation following laparotomy(CLP); the intervention group(group TL, n=24), underwent ligation of the thoracic catheter after CLP operation.The first two groups in the time points 3, 6, 12, 24 h was divided into 4 sub groups. Endotoxin concentration in thoracic duct and in portal vein were detected at 3h, 6h, 12 h and 24 h after operation of each group; and NF-kappa B P65 concentrations in lung tissue of each group were detected; lung histopathology HE staining, lung wet/dry weight ratio(W/D), apoptotic cells staining were also performed.The second part: the 54 clean level healthy male Wister rats were randomly divided into 3 groups: control group(SO group, n = 18), open after turning the cecum injection of saline solution; Experimental group(CLP group, n = 18), open the cecum ligation perforation after surgery(CLP); Intervention group(n = 18), CLP surgery preoperative or postoperative intraperitoneal injection sand chloride solution. Before the two groups in the phase 1 h, 2.5 h, 4 h respectively with preoperative intervention group(n=6): intraperitoneal injection of chlorine sand temple 1 h after CLP surgery, postoperative 0.5 h intervention group(n = 6) : line CLP 0.5h after surgery, postoperative intraperitoneal injection of chlorine sand 2 h intervention group(n = 6) : intraperitoneal injection of line 2 h after CLP surgery chlorine sand correspond. The control group, experimental group and intervention group was detected in the rat peritoneal lymphatic hole aperture size and distribution of density, thoracic duct lymph plasma endotoxin concentration, lung tissue NF-kappa B P65 immunohistochemical protein expression and apoptosis of protease Caspase3 relatively active. Results: 1.1The experimental group phase time points of thoracic duct lymph plasma endotoxin concentration is much higher than that of portal vein plasma(p<0.01); 1.2There were no significant difference of endotoxin concentration of portal vein at different time point in experimental group(p>0.05).;1.3the experimental group the same time phase, concentration of endotoxin of lymph and portal vein plasma were higher than those in the control group(p<0.05);1.4In the experimental group,the content of the lung tissue NF kappa B P65 pathological HE score, W/D, apoptosis index(AI) were higher than the control group in the same time points(p<0.05);1.5 the same time point content and the intervention group lung tissue of NF- kappa B P65 pathological HE score, W/D, AI were lower in the experimental group(p<0.05), but higher than that of the control group(p<0.05).2.1 the test group with the time prolonged, the aperture, the peritoneal lymphatic stomata density and thoracic duct lymph plasma endotoxin concentration gradually increased(p<0.05), the intervention group was significantly higher than that in the corresponding time points(P<0.01) and control group(P<0.01), while the intervention group was higher than that of the control group(p<0.05); preoperative intervention group of lymphatic stomata index and thoracic duct lymph plasma endotoxin content lower than postoperative intervention group(P<0.01), and after the 2h intervention group was higher than that of 0.5h after operation(P<0.05); 2.2The level of average optical density of lung tissue of NF-κB P65 and apoptotic protease Caspase3 activity in the preoperative intervention group is lower than after surgical intervention group(p<0.05), and 2h after operation were higher than those of the level of 0.5h(p<0.05); the intervention group were lower than the corresponding time point in experimental group, but higher than the level of the control group(p<0.05).Conclusion: Peritoneal lymphatic drainage plays an important role in endotoxin-induced lung injury during the early phase of abdominal infection, blockage of the peritoneal lymphatic drainage pathway could extenuate the severity of lung injury induced by endotoxin.2.1 the early abdominal cavity infection, AngⅡ-R inhibitor chlorine sand by regulating the peritoneal lymphatic hole the size of the aperture and density, reduce lung injury caused by endotoxin; 2.2 early abdominal cavity infection, drug intervention in the earlier, the lighter the degree of lung injury.
