| Objective:To observe the efficacy of 23-gauge minimally invasive vitrectomy (23-G MIV) for diabetic tractional retinal detachment (DTRD) with different vitreous substitutes.Methods:1. Clinical the data of 102 patients with DTRD received treatment at our hospital from 2011 to 2014 were retrospectively analyzed.2. All the 23-gauge micro-invasive vitrectomy operations performed by the same doctor.3. According to filling agent,102 patients were divided into sileicone oil taponade group (group SIO),inert-gas tamponade group(group C3F8) and BSS tamponade group (group BSS). All cases were followed up for 3-6 months. The efficacy of different treatments among three groups were analyzed.(inclouding the intraocular pressure/the visual acuity and the postoperative complications)Results:1. There were 102 cases of DTRD patients (102 eyes) were retrospective analyzed,including 22 in group SIO,42 in group BSS,38 in group C3F8,and 15 eyes had accepted implant intraocular lens implant.2. In the 102 eyes, the visual acuity (>0.02) composed of 48 eyes (47.1%) before operation to 73 eyes (71.6%) 2 month after surgery, of which the visual acuity (>0.1), increased from 14 eyes (13.7%) before operation to 33 eyes (32.4%);3. There were no significant differences among three groups of IOP before surgery,lmo and 3mo after surgery, but the IOP at 7d after surgery appears significant difference in group C3F8 compared with group BSS(P<0.05). All most of the high intraocular pressure were controlled by the medicine in one week.4. During 102 cases,36 cases (35.3%) develop complications.9 cases(40.9%) in group SIO,14 cases(36.8%) in group C3F8 and 13 cases(31.0%) in group BSS.Conclusion:23-gauge micro-invasive vitrectomy has good effect with the diabetic traction retinal detachment patients,as long as controlled the operations indications in strictly,accept the treatment in early, select of suitable vitreous substitutes and skilled in operating. |
