Long-term Efficacy Of Transarterial Chemoembolization And Transarterial Infusion Chemotherapy Followed By Transarterial Lipiodol Embolization In Patients With Hepatocellular Carcinoma:Contrastive Analysis

[Objective] To evaluate the long-term efficacy and the impact of transarterial chemoembolization (TACE) on the acute liver function damage with that of transarterial infusion chemotherapy followed by transarterial lipiodol embolization (TAI+TAE) in patients with hepatocellular carcinoma (HCC).[Method] The clinical data of 264 patients with unresectable hepatocellular carcinoma(HCC) treated by transarterial interventional treatment were retrospectively analyzed. Patients were divided into two groups:TACE group (n=136) accepted the interventional treatment for 445 times and TAI+TAE group (n=118) accepted the interventional treatment for 484 times. The overall survival (OS) and the liver function damage of the two groups had been observed before and 1 days latter after treatment retrospectively, including the clinical liver function grade change of the live function test (ALT,AST,TBIL,PALB,ALB).[Results] There were insignificant differences in the gender, age, accompanying hepatitis, Lesion type, distant metastasis, Portal venous emboli, ascites, Child-pugh gradebefore intervention therapy and serum AFP between the two groups (p>0.05),and no difference was found in the liver function biochemical indicators (ALT, AST, TBIL, PALB, ALB)before intervention therapy as well. Both of the two groups’liver function changed significantly after treatment (p<0.001), the transaminase and total bilirubin (ALT, AST, TBIL)level increased obviously and the prealbumin and albumin(PALB. ALB)level decreased. However, the change of ALT,AST,PALB(A ALt、△ AST、△ PALB) in TAI+TAE group is significantly higher than in tace group (p<0.05), but no significant differences on the index of the change of TBIL,ALB (△ TBIL、△ ALB) (p<0.05). the 1-, 2-,3-year cumulative OS rates and the median survival time (58.80%.39.00%.27.90% and 15 months respectively) in the TACE group was significantly higher than that in TAI+TAE group(50.80%.28.80%.17.80% and 12 months respectively),which reached statistical significance (p=0.026).[Conclusions] The long-term efficacy and the impact of TACE on the acute liver function damage were superior to that of TAI+TAE. TACE as a standard treatment for unresectable HCC compared with TAI+TAE demonstrated sufficient safety and reasonable efficacy to delay the progress of the cirrhosis of the liver and improve the long-term curative effect is of great significance.
