ObjectiveThere is controversy about the pathogenesis of Chronic Venous Insufficiency of the lower limbs all round the worldo There are gravity factor inflammation and vascular remodeling three theoryo In recent years inflammation theory has been widely recognized. This theory proposes that:CVI can be considered to represent an inflammatory response of leukocyte in the venous wall. The inflammatory factor such as TNFaand IL-6 play an important role. Since inflammation is the key to the occurrence and development of CVI, then intervention inflammation is an important link in prevention and treatment of CVI.Traditional Chinese medicine holds that CVI belong to "jinliu", "qingshedu", "lianchuang" category, it’s pathogenesis is Not Liver Freely Distributing:Stagnation of Liver-Qi、downward damp-heat、deficiency and wind dryness. Treatment of CVI is soothing liver, regulating Qi、activating blood to resolve stagnation, clearing away heat. Chai Hu Shu Gan San has all the effect.. The preliminary clinical study show that:Chai Hu Shu Gan San has a good treatment outcome in Chronic Venous Insufficiency of the lower limbs.So we put forward hypothesis that:Chai Hu Shu Gan San can inhibit to restrain inflammation and then help the Chronic Venous Insufficiency of the lower limbs to recoverd. We through the analysis the inflammatory factor TNFα and IL-6 level change and the clinical symptomson patients before and after medication. To provide the experimental data for Chai Hu Shu Gan San to treat the Chronic Venous Insufficiency of the lower limbs, and to provide the reference the development of Chinese medicine research.MethodsThis is a randomised controlled study. According to the CEAP classif icat- ion,divid 80 cases of voluntary into C3、C4、C5、C6, each group of 20 people. They are randomly divided into traditional Chinese medicine group and western medicine group. traditional Chinese medicine group receiving the treatment with Chai Hu Shu Gan San for 7 days, while the western medicine group receiving the treatment with Diosmin for 7 days. Detailed records patients clinical symptoms、TNF α and IL-6 level change、and the VCSS before and after taking the medicine.ResultsChai Hu Shu Gan San and Diosmin can ease the CVI patients clinical manifestations such as pain, swelling, inflammation, ulcer,p<0.05, with statistical significance. The can ease the C4、C5 patients clinical manifestations such as pigmentation、skin indurations,p<0.05, with statistical significance.The Chai Hu Shu Gan San and Diosmin can the VCSS, and the Chai Hu Shu Gan San Mean is lower than Diosmin. In the C3 patients p>0.05, without statistical significance;the C4、C5 patients,p<.0.05, with statistical significance.Chai Hu Shu Gan San and Diosmin make inflammatory factor such as TNF a and IL-6 declined significantly, p<0.05, respectively. ConeclusionChai Hu Shu Gan San can effectively relieve clinical symptoms of patients with Chronic Venous Insufficiency of the lower limbs, the pathogenesis is associated with inhibit to restrain inflammatory factor such as TNF a and IL-6.
